输尿管硬镜与软镜钬激光碎石术联合治疗复杂上尿路结石的临床疗效

摘　　要：目的探究输尿管硬镜与软镜钬激光碎石术联合治疗复杂上尿路结石的临床疗效。方法选取2018年5月至2020年5月新钢中心医院收治的50例复杂上尿路结石患者作为研究对象,按照随机数字表法分为两组,各25例。对照组采用输尿管硬镜钬激光碎石术治疗,观察组采用输尿管硬镜与软镜钬激光碎石术联合治疗,比较两组手术效果、生命质量、并发症发生率及复发率。结果观察组结石取净率(100.00%)高于对照组(76.00%),差异有统计学意义(P<0.05)。术后,两组36项简明健康调查表(SF-36)评分均高于术前,且观察组[(80.76±12.43)分]高于对照组[(71.19±10.26)分],差异有统计学意义(P<0.05)。观察组并发症总发生率(4.00%)及复发率(0.00%)均低于对照组(32.00%、28.00%),差异有统计学意义(P<0.05)。结论输尿管硬镜与软镜钬激光碎石术联合治疗复杂上尿路结石效果显著,能提高结石取净率,降低并发症发生率和复发率,提高患者生命质量,值得临床推广应用。Objective To explore the clinical efficacy of the combined rigid ureteroscope and flexible ureteroscope holmium laser lithotripsy in the treatment of complicated upper urinary tract stones.Methods 50 patients with complicated upper urinary tract stones treated in XinSteel Center Hospital from May 2018 to May 2020 were selected as the research subjects,and they were divided into two groups according to the random number table method,with 25 cases in each group.The control group was treated with rigid ureteroscope holmium laser lithotripsy,and the observation group was treated with the combined rigid ureteroscope and flexible ureteroscope holmium laser lithotripsy,the surgical effect,quality of life,incidence of complications and recurrence rate were compared between the two groups.Results The removal rate of stones of the observation group(100.00%)was higher than that of the control group(76.00%),and the difference was statistically significant(P<0.05).After operation,the 36-item short form health survey(SF-36)scores in the two groups were higher than those before operation,and the observation group([80.76±12.43]scores)was higher than the control group([71.19±10.26]scores),and the difference was statistically significant(P<0.05).The total incidence of complications(4.00%)and recurrence rate(0.00%)in the observation group were lower than those in the control group(32.00%,28.00%),and the difference was statistically significant(P<0.05).Conclusion The combined rigid ureteroscope and flexible ureteroscope holmium laser lithotripsy in the treatment of complicated upper urinary tract stones has significant effect,can increase the removal rate of stones,reduce the incidence of complications and recurrence rate,improve the quality of life of patients,which is worthy of clinical application.
