摘　　要：目的探讨曲安奈德+盐酸利多卡因+维生素B_(12)+0.9%氯化钠溶液混合液局部注射联合小针刀松解治疗神经根型颈椎病(CSR)的临床疗效。方法选取2018年8月至2020年11月于本院进行治疗的82例CSR患者作为研究对象,随机分为注射组和联合组,每组41例。注射组予以曲安奈德+盐酸利多卡因+维生素B_(12)+0.9%氯化钠溶液局部注射治疗,联合组在注射组基础上予以小针刀松解治疗。比较治疗1周后、1个月后及3个月后,两组临床疗效、视觉模拟评分法(VAS)评分、颈椎病临床评价量表(CASCS),比较治疗1个月后随访时不良反应发生率。结果治疗1周后及治疗1、3个月后,联合组治疗总有效率均明显高于注射组,VAS评分均低于注射组,CASCS评分均高于注射组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论曲安奈德+盐酸利多卡因+维生素B12+0.9%氯化钠溶液局部注射联合小针刀松解治疗CSR患者疗效显著,可缓解其症状,提高颈椎功能,值得临床推广应用。Objective To investigate the clinical efficacy of local injection of triamcinolone+lidocaine hydrochloride+vitamin B_(12)+0.9%sodium chloride solution combined with small needle knife release in the treatment of radiculopathy of cervical spondylopathy(CSR).Methods A total of 82 patients with CSR treated in our hospital from August 2018 to November 2020 were selected as the research subjects,and they were randomly divided into injection group and combined group,with 41 cases in each group.The injection group was treated with triamcinolone+lidocaine hydrochloride+vitamin B_(12)+0.9%sodium chloride solution by local injection,and the combined group was treated with small needle scalping on the basis of injection group.After 1 week of treatment,1 month of treatment and 3 months of treatment,the clinical efficacy,visual analog scale(VAS)score,clinical evaluation scale for cervical spondylosis(CASCS),and the incidence of adverse reactions were compared between the two groups.Results After 1 week of treatment and 1 and 3 months of treatment,the total effective rate of combined group was significantly higher than that of injection group,VAS score was lower than that of injection group,CASCS score was higher than that of injection group,the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups.Conclusion The local injection of triamcinolone+lidocaine hydrochloride+vitamin B_(12)+0.9%sodium chloride solution combined with small needle scalpectomy has significant efficacy in the treatment of CSR patients,which can relieve their symptoms and improve the function of cervical spine,and is worthy of clinical promotion and application.
