碎裂QRS波与急性心肌梗死患者心室重塑的相关性分析

摘　　要：目的探讨碎裂QRS波与急性心肌梗死(AMI)患者心室重塑的相关性。方法回顾性分析2019年1月至2020年6月本院收治的80例AMI患者的临床资料,统计心室重塑与碎裂QRS波发生情况,采用单因素检测分析性别、年龄、体重指数、合并症QRS波发生情况等因素对于AMI患者心室重塑的影响,分析碎裂QRS波与AMI患者心室重塑的相关性,并应用受试者工作特征曲线(ROC)曲线下面积(AUC)评价碎裂QRS波对AMI患者心室重塑的预测价值。结果80例AMI患者中发生心室重塑患者22例,发生率为27.50%(22/80);单因素分析结果显示,心室重塑的AMI患者碎裂QRS波发生率高于未发生心室重塑的AMI患者,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,AMI患者发生碎裂QRS波是AMI患者心室重塑的影响因素(OR>1,P<0.05);ROC曲线结果表明,碎裂QRS波用于预测AMI患者心室重塑的AUC为0.708,具有一定的预测价值,95%CI为0.580~0.837。结论碎裂QRS波发生率较高会对AMI患者心室重塑造成影响,故早期检测AMI患者的碎裂QRS波发生情况可预测AMI患者心室重塑的发生风险,对拟定临床早期治疗方案,改善患者预后具有积极意义。Objective To investigate the association of fractured QRS waves with ventricular remodeling in patients with acute myocardial infarction(AMI).Methods Clinical data from 80 AMI patients admitted to our hospital between January 2019 and June 2020 were retrospectively analyzed for ventricular remodeling and fragmentation QRS waves.The effect of single factor detection on ventricular remodeling in AMI patients was analyzed for sex,age,body mass index(BMI),comorbidities of QRS waves on ventricular remodeling in AMI patients,and the correlation of fragmentation QRS waves with ventricular remodeling in AMI patients was evaluated using the subjects'work profile curve(AUC).Results Among the 80 AMI patients,22 had ventricular remodeling,with an incidence of 27.50%(22/80).The incidence of broken QRS in AMI patients with ventricular remodeling was higher than that in AMI patients without ventricular remodeling,and the difference was statistically significant(P<0.05).Logistic regression analysis showed that the fragmentation of QRS wave in AMI patients was an influential factor for ventricular remodeling in AMI patients(OR>1,P<0.05).ROC curve results showed that the AUC of splintered QRS in predicting ventricular remodeling in AMI patients was 0.708,which had a certain predictive value,and 95%CI was 0.580-0.837.Conclusion The high incidence of splintered QRS will affect the ventricular remodeling in AMI patients,so early detection of the occurrence of splintered QRS in AMI patients can predict the risk of ventricular remodeling in AMI patients,and has positive significance for the formulation of early clinical treatment plan and improvement of the prognosis of patients.
