开放式腹膜前间隙无张力疝修补术在腹股沟疝中的应用效果及对并发症的影响

摘　　要：目的分析开放式腹膜前间隙无张力疝修补术在腹股沟疝中的应用效果及对并发症的影响。方法选取2020年1月—7月本院接诊且行传统张力疝修补术治疗的40例腹股沟疝患者作为对照组,另选取2020年8月至2021年1月本院接诊且行开放式腹膜前间隙无张力疝修补术治疗的40例腹股沟疝患者作为观察组,比较两组围手术期指标、疼痛评分(NRS评分)、并发症发生率。结果观察组术中出血量(29.84±5.97)ml、手术时间(49.36±3.83)min、恢复正常饮食时间(24.54±2.42)h、首次下床时间(19.32±3.37)h、住院时间(4.97±0.57)d均少于对照组(48.45±8.19)ml、(56.18±5.27)min、(41.38±4.98)h、(33.73±5.43)h、(6.83±1.14)d,差异有统计学意义(P<0.05);观察组术后24 h(3.24±1.03)分、术后48 h(2.87±0.77)分时的NRS评分均低于对照组(5.78±1.46)分、(4.52±1.16)分,差异有统计学意义(P<0.05);术后7 d,两组NRS评分比较差异无统计学意义;观察组1例并发症发生率为2.50%,低于对照组的20.00%,差异有统计学意义(P<0.05)。结论开放式腹膜前间隙无张力疝修补术治疗腹股沟疝患者效果显著,具有创伤小、术后恢复快、并发症少、疼痛感轻等优势,值得临床推广应用。Objective To analyze the effect of open tension free hernia repair in preperitoneal space on inguinal hernia and its influence on complications.Methods From January to July 2020,40 patients with inguinal hernia who were treated with traditional tension hernia repair in our hospital were selected as the control group.From August 2020 to January 2021,40 patients with inguinal hernia who were treated with open tension free hernia repair in the preperitoneal space in our hospital were selected as the observation group.Perioperative indicators,pain scores(NRS scores),and the incidence of complications were compared between the two groups.Results The amount of intraoperative bleeding(29.84±5.97)ml,the operation time(49.36±3.83)min,the time to return to normal diet(24.54±2.42)h,the time to get out of bed for the first time(19.32±3.37)h,and the hospital stay(4.97±0.57)d in the observation group were less than those in the control group(48.45±8.19)ml,(56.18±5.27)min,(41.38±4.98)h,(33.73±5.43)h,and(6.83±1.14)d,with significant differences(P<0.05);The NRS scores at 24 h(3.24±1.03)and 48 h(2.87±0.77)in the observation group were lower than those in the control group(5.78±1.46)and(4.52±1.16)(P<0.05);There was no significant difference in NRS scores between the two groups 7 days after operation;The complication rate of one case in the observation group was 2.50%,which was lower than that of the control group(20.00%),with a statistically significant difference(P<0.05).Conclusion Open tension free hernioplasty in the preperitoneal space is effective in the treatment of patients with inguinal hernia,with advantages such as less trauma,quick recovery,fewer complications,and less pain,which is worthy of clinical application.
