非抗生素康复疗法在脑卒中并发肺部多重耐药菌感染7例患者中的应用分析

摘　　要：目的观察非抗生素康复疗法在脑卒中并发肺部多重耐药菌(MDRO)感染患者中的应用效果。方法选择2019年10月至2020年6月在本院接受脑卒中后康复治疗的7例肺部MDRO感染患者为研究对象,统计分析患者治疗前和治疗2、4周后的外周血炎症介质水平、病灶吸收率和病原菌清除率。结果治疗前和治疗2、4周后,患者的白细胞计数(WBC)、中性粒细胞百分比(NEU%)、C反应蛋白(CRP)、降钙素原(PCT)水平比较,差异具有统计学意义(P<0.05);随着治疗时间的增加,患者的WBC、NEU%、CRP、PCT水平呈现出递减趋势(P<0.05)。治疗2周后,4例患者WBC水平与NEU%恢复正常,3例患者的CRP水平恢复正常,5例患者的PCT水平恢复正常,病灶吸收率为(56.71±18.26)%,病原菌清除率(41.41±12.86)%;治疗4周后,所有患者的WBC、NEU%、PCT均恢复正常,6例的CRP水平恢复正常,病灶吸收率为(87.29±11.28)%,病原菌清除率为(80.79±10.26)%。结论非抗生素康复疗法在脑卒中并发肺部MDRO感染治疗中效果显著,能有效抑制机体炎症反应,促进炎性病灶吸收和病原菌清除。Objective To observe the application effect of non-antibiotic rehabilitation therapy in patients with stroke complicated with pulmonary multi-drug resistant organism(MDRO)infection.Methods Seven patients with pulmonary MDRO infection who received post-stroke rehabilitation treatment in our hospital from October 2019 to June 2020 were selected as the research objects.The levels of peripheral blood inflammatory mediators,lesion absorption rate and pathogen clearance rate before treatment and 2 and 4 weeks after treatment were statistically analyzed.Results There were significant differences in white blood cell count(WBC),neutrophil percentage(NEU%),C-reactive protein(CRP)and procalcitonin(PCT)levels before treatment and 2 and 4 weeks after treatment(P<0.05);with the increase of treatment time,the levels of WBC,NEU%,CRP and PCT showed a decreasing trend(P<0.05).After 2 weeks of treatment,the WBC level and NEU%of 4 patients returned to normal,the CRP level of 3 patients returned to normal,the PCT level of 5 patients returned to normal,the lesion absorption rate was(56.71±18.26)%,and the pathogen clearance rate was(41.41±12.86)%;after 4 weeks of treatment,the WBC,NEU%and PCT of all patients returned to normal,the CRP level of 6 cases returned to normal,the lesion absorption rate was(87.29±11.28)%,and the pathogen clearance rate was(80.79±10.26)%.Conclusion Non-antibiotic rehabilitation therapy has a significant effect in the treatment of stroke complicated with pulmonary MDRO infection,which can effectively inhibit the body's inflammatory response and promote the absorption of inflammatory lesions and pathogen clearance.
