曲普瑞林反加疗法联合腹腔镜下囊肿剥除术治疗子宫内膜异位症的效果及对VEGF、IGF-1、sVCAM-1及sICAM-1水平的影响

摘　　要：目的探讨曲普瑞林反加疗法联合腹腔镜下囊肿剥除术治疗子宫内膜异位症的效果。方法选择2019年5月至2020年9月我院收治的150例子宫内膜异位症患者为研究对象,以随机数字表法将其分为对照组与观察组,各75例。对照组给予腹腔镜下囊肿剥除术,观察组在对照组基础上给予曲普瑞林反加疗法治疗。比较两组的治疗效果。结果观察组的治疗总有效率为96.00%,高于对照组的85.33%(P<0.05)。术后6个月,观察组的血管内皮生长因子(VEGF)、胰岛素样生长因子-1(IGF-1)、可溶性血管细胞黏附分子-1(sVCAM-1)、可溶性细胞间黏附分子-1(sICAM-1)水平低于对照组(P<0.05)。观察组的不良事件总发生率为2.67%,低于对照组的16.00%(P<0.05)。结论曲普瑞林反加疗法联合腹腔镜下囊肿剥除术治疗子宫内膜异位症的效果较好,可有效调节VEGF、IGF-1、sVCAM-1及sICAM-1水平,且安全性高,值得推广。Objective To explore the effect of triptorelin add-back therapy combined with laparoscopic cystectomy in the treatment of endometriosis.Methods A total of 150 patients with endometriosis admitted in our hospital from May 2019 to September 2020 were selected as the research objects and divided into control group and observation group by random number table method,with 75 cases in each group.The control group was treated with laparoscopic cystectomy,and the observation group was treated with triptorelin add-back therapy on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 96.00%,which was higher than 85.33%in the control group(P<0.05).Six months after operation,the levels of vascular endothelial growth factor(VEGF),insulin-like growth factor-1(IGF-1),soluble vascular cell adhesion molecule-1(sVCAM-1)and soluble intercellular adhesion molecule-1(sICAM-1)in the observation group were lower than those in the control group(P<0.05).The total incidence of adverse events in the observation group was 2.67%,which was lower than 16.00%in the control group(P<0.05).Conclusion Triptorelin add-back therapy combined with laparoscopic cystectomy is effective in the treatment of endometriosis,it can effectively regulate the levels of VEGF,IGF-1,sVCAM-1 and sICAM-1,and has high safety,which is worthy of promotion.
