摘  要:目的探讨针刺联合肌电生物反馈治疗脑卒中偏瘫的效果及对肢体功能重建的影响。方法选取2020年5月至2021年4月我院收治的110例脑卒中偏瘫患者为研究对象,随机将其分为对照组和观察组,各55例。两组均给予常规治疗及康复训练,对照组给予肌电生物反馈治疗,观察组在对照组基础上给予针刺治疗。比较两组的治疗效果。结果治疗后,两组的口眼歪斜、偏瘫、神志昏蒙及语言謇涩评分均降低,且观察组低于对照组(P<0.05)。治疗后,两组的上、下肢最大积分肌电值(iEMG)均升高,且观察组高于对照组(P<0.05)。治疗后,两组的中枢神经特异性蛋白β(S100β)、神经元特异性烯醇化酶(NSE)水平均降低,神经生长因子(NGF)、脑源性神经营养因子(BDNF)水平均升高,且观察组优于对照组(P<0.05)。治疗后,两组的手臂动作调查测试(ARAT)、Fugl-Meyer运动功能评定量表(FMA)、躯干控制测试(TCT)、功能独立性评定量表(FIM)评分均升高,且观察组高于对照组(P<0.05)。结论针刺联合肌电生物反馈治疗脑卒中偏瘫可改善患者中医证候积分及神经细胞因子水平,促进肢体功能重建,值得临床推广。Objective To explore the effect of acupuncture combined with electromyographic biofeedback on stroke hemiplegia and its influence on limb function reconstruction.Methods A total of 110 stroke hemiplegia patients admitted in our hospital from May 2020 to April 2021 were selected as the research objects and randomly divided into control group and observation group,with 55 cases in each group.Both groups were given routine treatment and rehabilitation training,the control group was given electromyographic biofeedback treatment,and the observation group was given acupuncture treatment on the basis of the control group.The therapeutic effects of the two groups were compared.Results After treatment,the scores of mouth and eye deviation,hemiplegia,confusion and language astringency in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,the integrated electromyography(iEMG)of upper and lower limbs in the two groups increased,and those in the observation group were higher than the control group(P<0.05).After treatment,the levels of central nervous system specific proteinβ(S100β)and neuron-specific enolase(NSE)in the two groups decreased,while the levels of nerve growth factor(NGF)and brain-derived neurotrophic factor(BDNF)increased,and those in the observation group were better than the control group(P<0.05).After treatment,the scores of Action Research Arm Test(ARAT),Fugl-Meyer Assessment(FMA),Trunk Control Test(TCT)and Functional Independence Measure(FIM)in the two groups increased,and those in the observation group were higher than the control group(P<0.05).Conclusion Acupuncture combined with electromyographic biofeedback in the treatment of stroke hemiplegia can improve the TCM syndrome scores and nerve cell factors levels of patients,and promote the reconstruction of limb function,which is worthy of clinical promotion.
