改良式子宫下段缩窄缝合术治疗完全性前置胎盘的效果

摘　　要：目的探讨对完全性前置性胎盘产妇使用改良式子宫下段缩窄缝合手术进行治疗的效果。方法选择2020年3月至2022年9月我院收治的80例完全性前置胎盘产妇作为研究对象,利用随机数表法将产妇分为两组,分别为研究组和对照组,每组40例。对照组使用子宫下段螺旋式缝合手术进行治疗,研究组则使用改良式子宫下段缩窄缝合术进行治疗。在治疗结束后,比较两组产妇治疗后的出血量、手术时间、术后3 d凝血纤维蛋白溶解指标、术后2 d的人胎盘生乳素(hPL)-mRNA和甲胎蛋白(AFP)水平、产妇产后的恢复指标、并发症发生率等。结果相比于对照组患者,研究组患者出血量、手术时间、术后3 d的凝血纤维蛋白溶解指标、术后2 d的hPL-mRNA和AFP水平、产妇产后恢复指标均更优,差异有统计学意义(P<0.05)。结论对完全性前置性胎盘产妇使用改良式子宫下段缩窄缝合手术进行治疗有助于提高产妇产后恢复速度,降低产妇并发症的发生概率。Objective To analyze and discuss the effect and value of modified lower segment constriction suture for the treatment of complete placenta previa.Methods A total of 80 pregnant women with complete placenta previa admitted to our hospital from March 2020 to September 2022 were selected as the subjects of this study,and the pregnant women were randomly divided into two groups,the study group and the control group,with 40 pregnant women in each group.The control group treated the women with complete placenta previa with spiral suture of the lower segment of the uterus,while the research group treated the women with complete placenta previa with modified lower segment constriction suture.After treatment,the bleeding volume,operation time,the blood coagulation and fibrinolysis level index of 3 days,the level index of hPL mRNA and AFP level index of 2 days after operation,the recovery index of postpartum women,the total complication rate between two groups were compared.Results Compared with the patients in the control group,the patients in the study group had better clinical bleeding volume and operation time,blood coagulation and fibrinolysis level indicators in 3 days,hPL mRNAand AFP level indicators in 2 days after operation,and postpartum recovery indicators of the puerpera,the differences were statistically significant(P<0.05).Conclusion The treatment of the puerpera with complete placenta previa with modified lower uterine segment constriction and suture can improve the postpartum recovery index of the puerpera and reduce the probability of complications of the puerpera.
