大承气汤联合隔药灸治疗结肠癌术后肠梗阻临床观察

摘　　要：目的探讨大承气汤加味联合隔药灸神阙八阵穴对结肠癌术后肠梗阻患者胃肠功能的影响及临床疗效。方法将78例结肠癌术后肠梗阻患者随机分为对照组(39例)和试验组(39例)。对照组给予阿莫西林、生长抑素及对症治疗,试验组在对照组基础上给予大承气汤加味联合隔药灸神阙八阵穴治疗,2组均治疗2周。比较2组的临床疗效、胃肠功能指标及血清二胺氧化酶(DAO)、胃动素(MTL)、肿瘤坏死因子-α(TNF-α)及白细胞介素-8(IL-8)的含量。结果试验组总有效率94.87%(37/39)高于对照组的76.92%(30/39)(P<0.05)。与对照组比较,试验组治疗后症状消失时间、肠鸣音恢复时间、排气恢复时间、进食恢复时间、总住院时间较短,血清DAO、TNF-α、IL-8含量较低,MTL含量较高(P<0.05)。结论大承气汤加味联合隔药灸神阙八阵穴治疗结肠癌术后肠梗阻患者的临床效果显著,能有效促进胃肠功能恢复,减轻炎症反应。Objective To investigate the clinical effect and influence of Dachengqi decoction combined with medicine-separated moxibustion at Shenque Bazhen points on gastrointestinal function in patients with postoperative intestinal obstruction of colon cancer.Methods 78 patients with postoperative intestinal obstruction of colon cancer were randomly divided into control group of 39 cases and treatment group of 39 cases.The control group was treated with amoxicillin,somatostatin and symptomatic treatment,and the treatment group was treated with modified Dachengqi decoction combined with medicine-separated moxibustion at Shenque Bazhen points on the basis of the control group.The two groups were both treated for 2 weeks.The clinical effect,gastrointestinal function indexes,serum diamine oxidase(DAO),motilin(MTL),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)contents were compared between the two groups.Results The total effective rate of the experimental group was 94.87%(37/39),which was higher than that of the control group(76.92%,30/39)(P<0.05).Compared with the control group,the duration of symptom disappearance,bowel sound recovery,exhaust recovery,feeding recovery and total hospital stay in the experimental group after treatment were shorter,the serum contents of DAO,TNF-αand IL-8 were lower,and the MTL content was higher(P<0.05).Conclusion Dachengqi decoction combined with medicine-separated moxibustion at Shenque Bazhen points has significant clinical effect in the treatment of patients with postoperative intestinal obstruction of colon cancer,which can effectively promote the recovery of gastrointestinal function and reduce inflammatory reaction.
