加味当归四逆汤联合铺灸与灌肠治疗寒湿瘀滞型慢性盆腔痛临床观察

摘　　要：目的观察加味当归四逆汤配合灌肠加铺灸,内外兼用治疗寒湿瘀滞型慢性盆腔痛的临床效果。方法收集寒湿瘀滞型慢性盆腔痛病例60例,随机分成治疗组和对照组,治疗组采取加味当归四逆汤口服,联合下腹及八髎穴铺灸和中药保留灌肠进行治疗,对照组采用广谱抗生素抗炎对症治疗。出院后随访6个月,对其效果进行评估分析,并对中医证候评分及疼痛简易评分进行比较分析。结果治疗组主症评分(10.61±2.68)分改善为(4.43±3.65)分,次症评分(17.67±3.65)分改善为(6.82±5.56)分,盆腔疼痛评分(5.42±1.59)分改善为(2.28±2.09)分;对照组主症评分(10.53±2.83)分改善为(6.71±3.68)分,次症评分(17.87±4.13)分改善为(10.04±5.52)分,盆腔疼痛评分(5.37±1.64)分改善为(3.57±2.14)分。2组比较,差异均有统计学意义(P<0.05)。治疗组总有效率为93.33%(28/30),高于对照组的73.33%(22/30),差异有统计学意义(P<0.05)。结论当归四逆汤联合铺灸、灌肠治疗寒湿瘀滞型慢性盆腔痛效果肯定。Objective To observe the clinic curative effect of modified Danggui Sini decoction combined with snake-like moxibustion and enema in the treatment of chronic pelvic pain of cold-dampness stagnation type.Methods Sixty cases of chronic pelvic pain of colddampness stagnation type were randomly divided into treatment group and control group.The treatment group was treated with Danggui Sini decoction combined with lower abdomen and Baliao point snake-like moxibustion and Chinese medicine retention enema,while the control group was treated with broad-spectrum antibiotic and anti-inflammatory symptomatic treatment.Following-up for 6 months after discharge was carried out to evaluate and analyze the effect,traditional Chinese medicine syndrome score and pain simple score between the two groups was compared.Results In the treatment group,the main disease score(10.61±2.68)was improved to(4.43±3.65)points,the secondary disease score(17.67±3.65)was improved to(6.82±5.56)points,and the pelvic pain score(5.42±1.59)was improved to(2.28±2.09)points.In the control group,the main disease score(10.53±2.83)was improved to(6.71±3.68),the secondary disease score(17.87±4.13)was improved to(10.04±5.52),and the pelvic pain score(5.37±1.64)was improved to(3.57±2.14).There was statistical significance between the two groups(P<0.05).The total effective rate of the treatment group was 93.33%(28/30),which was higher than that of the control group(73.33%,22/30),and the difference was statistically significant(P<0.05).Conclusion Danggui Sini decoction combined with snake-like moxibustion and enema in the treatment of chronic pelvic pain with cold and dampness stasis is positive.
