针刺联合化痰通络解痉汤治疗脑卒中后痉挛性瘫痪临床观察

摘　　要：目的探讨针刺联合化痰通络解痉汤治疗脑卒中后痉挛性瘫痪的临床效果。方法将2019年6月—2021年4月鞍山市中心医院收治的74例脑卒中后痉挛性瘫痪患者随机分成对照组和研究组,各37例。2组均接受常规康复训练,对照组给予针刺疗法,研究组给予针刺联合化痰通络解痉汤,持续4周。比较2组的中医症状积分、阿什沃思(Ashworth)量表评分和Fugl-Meyer运动功能评定量表(FMA)评分。结果治疗4周后,2组中医症状积分低于治疗前(P<0.05),且研究组中医症状积分低于对照组(P<0.05);2组Ashworth量表评分均明显低于治疗前,上肢运动功能评分、下肢运动功能评分和FMA总分明显高于治疗前(P<0.05),且研究组Ashworth量表评分、FMA评分均优于对照组(P<0.05)。结论针刺联合化痰通络解痉汤能明显降低脑卒中后痉挛性瘫痪患者的中医症状积分,减轻痉挛程度和改善肢体运动功能。Objective To explore the therapeutic effect of acupuncture combined with Huatan Tongluo Jiejing decoction on spastic paralysis after stroke.Methods 74 patients with spastic paralysis after stroke treated in Anshan Central Hospital from June 2019 to April 2021 were randomly divided into control group and study group.Both groups received routine rehabilitation training,the control group was given acupuncture therapy,and the study group was given acupuncture combined with Huatan Tongluo Jiejing decoction for 4 weeks.The traditional Chinese medicine(TCM)syndrome score,Ashworth scale and Fugl Meyer(FMA)score were compared between the two groups.Results After 4 weeks of treatment,the TCM syndrome score of 2 groups was lower than before treatment(P<0.05),and the TCM syndrome score of the study group was lower than that of the control group(P<0.05).Ashworth scale scores of the 2 groups were significantly lower than before treatment,upper limb motor function scores,lower limb motor function scores and total FMA scores were significantly higher than before treatment(P<0.05),and Ashworth scale scores and FMA scores of the study group were better than those of the control group(P<0.05).Conclusion Acupuncture combined with Huatan Tongluo Jiejing decoction can significantly reduce the TCM syndrome score of patients with spastic paralysis after stroke,reduce the degree of spasticity,and improve limb motor function.
