摘 要:目的:观察丁二磺酸腺苷蛋氨酸联合熊去氧胆酸治疗人巨细胞病毒(HCMV)感染所致新生儿肝炎综合征的效果。方法:选取2018年7月至2020年12月该院收治的86例HCMV感染所致的新生儿肝炎综合征患儿进行前瞻性研究,以随机数字表法将其分为对照组(n=43)和观察组(n=43)。对照组行熊去氧胆酸治疗,观察组在对照组基础上联合丁二磺酸腺苷蛋氨酸治疗,比较两组疗效、治疗前后肝功能指标[丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)、总胆汁酸(TBA)和天门冬氨酸氨基转移酶(AST)]水平、炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]水平、治疗期间不良反应发生率和恢复时间。结果:观察组治疗总有效率为93.02%(40/43),高于对照组的74.42%(32/43),差异有统计学意义(P<0.05);治疗后,两组ALT、TBIL、TBA、AST、TNF-α和IL-6水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);随访6周,观察组恢复时间短于对照组,差异有统计学意义(P<0.05)。结论:丁二磺酸腺苷蛋氨酸联合熊去氧胆酸治疗HCMV感染所致新生儿肝炎综合征可降低肝功能指标和炎性因子水平,缩短恢复时间,提高治疗总有效率,效果优于单纯熊去氧胆酸治疗。Objective:To observe effects of Ademetionine 1,4-butanedisulfonate combined with Ursodeoxycholic acid in treatment of neonatal hepatitis syndrome caused by human cytomegalovirus(HCMV)infection.Methods:A prospective study was conducted on 86 children with neonatal hepatitis syndrome caused by HCMV infection admitted to the hospital from July 2018 to December 2020.They were divided into control group(n=43)and observation group(n=43)by using the random number table method.The control group was treated with Ursodeoxycholic acid,while the observation group was treated with Ademetionine 1,4-butanedisulfonate on the basis of that of the control group.The therapeutic effects,the liver function index levels[alanine aminotransferase(ALT),total bilirubin(TBIL),total bile acid(TBA)and aspartate aminotransferase(AST)],the inflammatory factor levels[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]before and after the treatment,the incidence of adverse reactions during the treatment,and the recovery time were compared between the two groups.Results:The total effective rate of treatment in the observation group was 93.02%(40/43),which was higher than 74.42%(32/43)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of ALT,TBIL,TBA,AST,TNF-αand IL-6 in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Further,after 6 weeks of follow-up,the recovery time of observation group was shorter than that of control group,and the difference was statistically significant(P<0.05).Conclusions:Ademetionine 1,4-butanedisulfonate combined with Ursodeoxycholic acid in the treatment of neonatal hepatitis syndrome caused by HCMV infection can reduce the levels of liver function indexes and inflammatory factors,shorten the recovery time and improv
