摘  要:目的:观察医学营养治疗在妊娠期糖尿病患者中的应用效果。方法:选取2021年3月至2022年10月该院收治的156例妊娠期糖尿病患者进行前瞻性研究,按照随机数字表法分为观察组与对照组各78例。对照组给予常规治疗,观察组在对照组基础上采用医学营养治疗。比较两组血糖[空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)]水平、孕期总增重、孕期增重异常发生率、遵医率和不良妊娠结局发生率。结果:产前1周,两组FPG、2hPG、HbA1c水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组孕期总增重和孕期增重异常发生率均低于对照组,差异有统计学意义(P<0.05);观察组按时复查、合理运动、血糖监测、饮食控制遵医率均高于对照组,差异有统计学意义(P<0.05);观察组不良妊娠结局发生率为2.56%,明显低于对照组的21.79%,差异有统计学意义(P<0.05)。结论:在常规治疗基础上采用医学营养治疗可降低妊娠期糖尿病患者血糖水平、孕期总增重、孕期增重异常发生率和不良妊娠结局发生率,提高其遵医率,效果优于单纯常规治疗。Objective:To observe effects of medical nutrition therapy on patients with gestational diabetes mellitus.Methods:A prospective study was conducted on 156 patients with gestational diabetes mellitus admitted to this hospital from March 2021 to October 2022.They were divided into observation group and control group according to the random number table method,78 cases in each.The control group was given routine treatment,while the observation group was treated with medical nutrition therapy on the basis of that of the control group.The levels of blood glucose[fasting blood glucose(FPG),2 h postprandial blood glucose(2hPG),glycosylated hemoglobin(HbA1c)],the total weight gain during pregnancy,the incidence of abnormal weight gain during pregnancy,the compliance rate,and the incidence of adverse pregnancy outcomes were compared between the two groups.Results:1 week before delivery,the levels of FPG,2hPG and HbA1c in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total weight gain during pregnancy and the incidence of abnormal weight gain during pregnancy in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The compliance rates of timely review,reasonable exercise,blood glucose monitoring and diet control in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of adverse pregnancy outcomes in the observation group was 2.56%,which was significantly lower than 21.79%in the control group,and the difference was statistically significant(P<0.05).Conclusions:On the basis of the routine treatment,the medical nutrition therapy can reduce the blood glucose levels,total weight gain during pregnancy,the incidence of abnormal weight gain during pregnancy and the incidence of adverse pregnancy outcomes in the patients wit
