基于数据挖掘探讨王行宽辨治冠状动脉粥样硬化性心脏病合并高血压病遣方用药规律

摘　　要：目的应用中医传承计算平台(V3.0)探讨王行宽教授治疗冠心病合并高血压的遣方用药规律。方法纳入王行宽教授治疗冠心病合并高血压门诊病历录入中医传承计算平台进行数据挖掘与分析。结果获得有效处方共346首,涉及药物147味,临床用药药性以寒、温为主,药味以甘、苦为主,归经多归肝经,使用频次较高的前10位药物有白蒺藜、天麻、钩藤等,主要治法是平肝潜阳、豁痰化瘀、疏肝利胆、益气养营,演化出6组药物核心组合。结论王行宽教授治疗冠心病合并高血压用药上寒温并用、精简平和,常用平肝煎合生脉散合柴胡陷胸汤加减,主要治法是平肝潜阳、豁痰化瘀、疏肝利胆、益气养营,治疗上倡导“间者并行”“肝心同治”,并由此推测出王老认为冠心病合并高血压的病性属本虚标实,病机是心之气营两虚为本,痰瘀互结、肝阳亢盛为标。Objective To explore the regularity of Professor Wang Xingkuan's prescription for the treatment of coronary heart disease with hypertension using the Traditional Chinese Medicine Inheritance Computing Platform(V3.0).Methods Professor Wang Xingkuan's outpatient medical records for treating coronary heart disease with hypertension were entered into the traditional Chinese medicine inheritance computing platform for data mining and analysis.Results A total of 346 effective prescriptions were obtained,involving 147 flavors of drugs.The clinical drug properties were mainly cold and warm,and the drug flavors were mainly sweet and bitter.The meridians were mostly attributed to the liver meridians.The top 10 drugs with higher frequency of use were tribulus terrestris,gastrodia elata,and uncaria.The main treatment methods were to calm the liver and dissipate yang,eliminate phlegm and blood stasis,dredge the liver and promote the gallbladder,and nourish qi and nourish the camp.Six core combinations of drugs were evolved.Conclusion Professor Wang Xingkuan's treatment of coronary heart disease combined with hypertension is based on the combination of cold and warm,simplified and peaceful medication,commonly used in combination with Pinggan Decoction(平肝煎),Shengmai Powder(生脉散),and Chaihu Xianxiong Decoction(柴胡陷胸汤).The main treatment methods are to calm the liver and dissipate yang,remove phlegm and remove stasis,dredge the liver and promote the gallbladder,and nourish qi and nourish the camp.In terms of treatment,he advocates"parallel treatment between the two",and"simultaneous treatment of the liver and the heart".It is speculated that Wang believes that the pathogenesis of coronary heart disease combined with hypertension is based on the deficiency of both qi and blood,and the pathogenesis is based on the deficiency of both qi and blood in the,Phlegm and blood stasis intermingle and hyperactivity of liver yang are the criteria.
