摘 要:目的基于Akt/mTOR及Beclin-1信号通路探讨清金化痰汤对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)痰热郁肺型大鼠作用机制的研究。方法将造模成功后的大鼠随机分为模型组、罗红霉素组(0.0315 g·kg^(-1))、清金化痰汤组(18.8 g·kg^(-1)),每组各8只。另取8只正常大鼠作为空白对照组,空白对照组给予给予等体积的生理盐水,1次/d,连续给药2周。采用脂多糖气管注射+烟熏法建立痰热郁肺型大鼠模型,依据大鼠生物学体征变化和肺组织形态学改变作为模型成功的判定标准。观察各组大鼠肺组织病理变化,免疫组化检测各组大鼠肺组织Akt、mTOR、Beclin-1蛋白表达水平,qRT-PCR检测各组大鼠肺组织Akt、mTOR、Beclin-1 mRNA表达。结果清金化痰汤组大鼠肺组织支气管壁明显变薄,炎症细胞浸润减少,充血水肿有所减轻。与空白对照组比较,模型组大鼠肺组织中Akt、mTOR蛋白和mRNA表达明显降低(P<0.01);与模型组比较,清金化痰汤组大鼠肺组织中Akt、mTOR蛋白和mRNA表达均明显升高(P<0.01)。与空白对照组比较,模型组大鼠肺组织中Beclin-1蛋白和mRNA表达明显升高(P<0.01);与模型组比较,清金化痰汤组大鼠肺组织中Beclin-1蛋白和mRNA表达均明显降低(P<0.01)。结论清金化痰汤可在一定程度上改善COPD痰热郁肺型大鼠肺组织炎性病变,同时可能通过上调Akt、mTOR蛋白和mRNA的表达,降低Beclin-1蛋白和mRNA表达,从而减轻COPD痰热郁肺型大鼠气道炎症。Objective To explore the mechanism of Qingjin Huatan Decoction on COPD rats with phlegm heat stagnation of lung based on Akt/mTOR and beclin-1 signaling pathway.Methods The rats were randomLy divided into model group,roxithromycin group(0.0315 g·kg^(-1))and Qingjin Huatan Decoction group(18.8 g·kg^(-1)),with 8 rats in each group.Another 8 normal rats were taken as the blank control group.The blank control group was given equal volume of normal saline,once a day for 2 weeks.The rat model of phlegm heat stagnation in lung was established by lipopolysaccharide intratracheal injection and smoking.The changes of biological signs and lung histomorphology were used as the criteria for the success of the model.The pathological changes of lung tissue in each group were observed,the expression levels of Akt,mTOR and beclin-1 protein in lung tissue were detected by immunohistochemistry,and the expression levels of Akt,mTOR and beclin-1 mRNA in lung tissue were detected by QRT-PCR.Results The pathological morphology of lung tissues in Qingjin Huatan Decoction group was significantly thinner,the infiltration of inflammatory cells was reduced,and the congestion and edema were alleviated.Compared with the blank control group,the expression of Akt,mTOR protein and mRNA in the lung tissue of the model group decreased significantly(P<0.01),compared with the model group,the expressions of Akt,mTOR protein and mRNA in lung tissue of rats in Qingjin Huatan Decoction group were significantly increased(P<0.01).Compared with the blank control group,the expression of beclin-1 protein and mRNA in the lung tissue of model group increased significantly(P<0.01),compared with the model group,the expression of beclin-1 protein and mRNA in lung tissue of rats in Qingjin Huatan Decoction group decreased significantly(P<0.01).Conclusion Qingjin Huatan Decoction can improve the inflammatory lesions of lung tissue in COPD rats with phlegm heat stagnation of lung to a certain extent.At the same time,it may reduce the expression of beclin-1 protein
