河北省健康胎儿脐带血血常规红细胞参数参考范围的研究与建立

摘　　要：目的建立河北省妊娠期胎儿脐带血血常规红细胞参数的参考范围,并探讨孕周、性别与胎儿红细胞参数参考范围变化的关系。方法收集河北省人民医院2010年5月至2021年7月因各种指征行产前诊断随访结果均正常的3069例孕18~38周的胎儿脐带血,进行脐带血血常规检测,根据不同孕周对血常规红细胞计数、血红蛋白浓度、红细胞压积、平均红细胞体积、平均红细胞血红蛋白量、平均红细胞血红蛋白浓度的检测结果进行汇总分析。结果胎儿的血常规红细胞各参数结果呈非正态分布。随着孕周增加红细胞计数、血红蛋白、红细胞压积、平均血红蛋白浓度逐渐升高,红细胞平均体积、平均血红蛋白含量逐渐降低。不同性别胎儿红细胞计数、血红蛋白、平均血红蛋白浓度、红细胞平均体积、平均血红蛋白含量的总体差异具有统计学意义(P<0.05)。结论河北省22~38周健康胎儿脐带血血细胞参数在不同孕周及性别之间存在差异,应该独立制订河北省妊娠期胎儿的血细胞参数参考范围,有助于胎儿产前和早产儿疾病的诊断。Objective To establish the reference range of routine red blood cell parameters in fetal umbilical cord blood during pregnancy in Hebei province,and to investigate the correlation between gestational week,gender and the change of reference range of fetal red blood cell parameters.Methods A total of 3069 umbilical cord blood samples from fetuses at 18-38 weeks of gestation with normal prenatal diagnosis and follow-up results due to various indices admitted to Hebei General Hospital from May 2010 to July 2021 were collected,and the umbilical cord blood routine test was carried out.According to different gestational weeks,the test results of routine red blood cell count,hemoglobin concentration,hematocrit,average red blood cell volume,average red blood cell hemoglobin content and average red blood cell hemoglobin concentration were summarized and analyzed.Results The results of red blood cell parameters in the fetal blood routine showed a non-normal distribution.With the increase of gestational weeks,the red blood cell count,hemoglobin,hematocrit and average hemoglobin concentration gradually increased,while the average volume and average hemoglobin content of red blood cells gradually decreased.The overall differences in fetal red blood cell count,hemoglobin,average hemoglobin concentration,average red blood cell volume and average hemoglobin content between different genders were statistically significant(P<0.05).Conclusion There are differences in blood cell parameters of umbilical cord blood of healthy fetuses from 22 to 38 weeks in Hebei Province between different gestational weeks and genders.The reference range of blood cell parameters of pregnant fetuses in Hebei Province shall be independently established,which is helpful for the diagnosis o f prenatal and premature diseases in fetuses.
