外侧锁定钢板联合内侧支持钢板与双支持钢板用于复杂胫骨平台骨折的疗效比较

摘　　要：目的探讨复杂胫骨平台骨折患者行外侧锁定钢板联合内侧支持钢板与双支持钢板的治疗效果及预后情况。方法于2018年6月至2022年6月在钦州市第一人民医院采集病例展开研究,以严格按照纳排标准筛选后的60例复杂胫骨平台骨折患者为研究对象,基于“简单随机化方法”分为对照组与观察组,每组各30例,对照组实施双支持钢板固定治疗,观察组实施外侧锁定钢板联合内侧支持钢板固定治疗。统计手术相关指标,检测手术前后的影像学参数(胫骨平台内翻角、后倾角),利用HSS评分表判定膝关节功能恢复情况,观察统计术后并发症情况,对以上两组数据进行比较。结果观察组患者骨折愈合时间、完全负重时间短于对照组,差异有统计学意义(P<0.05)。术前、术后,两组患者的胫骨平台内翻角、后倾角组内、组间比较,差异无统计学意义(P>0.05)。术前两组患者关节功能HSS评分比较,差异无统计学意义(P>0.05);术后两组患者HSS评分均高于同组术前,且观察组患者高于对照组,差异有统计学意义(P<0.05)。两组患者术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论复杂胫骨平台骨折治疗中实行外侧锁定钢板联合内侧支持钢板,相比双支持钢板固定,在促进骨折愈合、早期负重活动方面更具优势,有利于膝关节功能恢复,临床疗效确切。Objective To investigate the efficacy and prognosis of lateral locking plate combined with medial support plate versus double support plate in patients with complex tibial plateau fractures.Methods A total of 60 patients with complex tibial plateau fractures in The First People’s Hospital of Qinzhou from June 2018 to June 2022 were screened based on strict inclusion and exclusion criteria,and were divided into a control group(n=30)and an observation group(n=30)by using the simple randomization method.The control group was treated with double support plate fixation,while the observation group was treated with lateral locking plate combined with medial support plate fixation.Surgery-related indicators were counted,pre-and post-surgery imaging parameters(inversion angle and posterior slope angle of the tibial plateau)were tested,recovery of knee joint function was determined by hospital for special surgery(HSS)knee score,and post-surgery complications were observed and counted.The above data were also compared between the two groups.Results The healing time of fractures and the complete loading time were shorter in the observation group than in the control group,with statistically significant differences(P<0.05).There were no statistically significant differences in terms of inversion angle and posterior slope angle of the tibial plateau in intra-group and inter-group comparisons before and after surgery(P>0.05).Before surgery,there was no statistically significant difference between the two groups in HSS knee scores(P>0.05).After surgery,HSS knee scores in both groups were higher than those before surgery,and those in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups in the total incidence of post-surgery complications(P>0.05).Conclusion Compared with double support plate fixation,lateral locking plate combined with medial support plate is more effective and practical in prom
