摘　　要：目的 观察孟鲁司特钠联合甲泼尼龙治疗儿童哮喘的效果。方法 本研究为前瞻性病例对照研究。选取2020年5月—2021年6月南京医科大学附属苏州医院收治的116例哮喘患儿作为研究对象,根据就诊单号尾数的奇偶数分为对照组(n=58)和研究组(n=58)。在常规治疗基础上,对照组患儿采用甲泼尼龙持续治疗10 d,研究组患儿在对照组基础上联合孟鲁司特钠持续治疗10 d。比较2组临床疗效,治疗前及治疗10 d后第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC、呼气流量峰值(PEF)、信号转导子和转录激活子6(STAT-6)蛋白相对表达量及临床症状改善时间。结果 研究组总有效率高于对照组(96.55%vs. 86.21%,χ^(2)=3.940,P=0.047)。治疗10 d后,2组FEV_(1)、FVC、FEV_(1)/FVC、PEF高于治疗前,STAT-6蛋白相对表达量低于治疗前,且研究组升高/降低幅度大于对照组(P<0.05或P<0.01)。研究组咳嗽、气喘、呼吸困难改善时间短于对照组(P<0.01)。结论 儿童哮喘采用孟鲁司特钠联合甲泼尼龙治疗的效果较佳,能够有效改善患儿的肺功能,快速减轻患儿的临床症状。Objective To investigate the effect of montelukast sodium combined with methylprednisolone for children with asthma.Methods This study is a prospective case-control study.A total of 116 children with asthma treated in Suzhou Hospital Affiliated to Nanjing Medical University from May 2020 to June 2021 were selected,and they were divided into the control group(n=58)and the study group(n=58)according to the odd and even numbers of the last number of the children′s visit list.Based on the conventional therapy,the control group was treated with methylprednisolone for 10 days,the study group was treated with montelukast sodium for 10 days based on the control group.Clinical effect,FEV_(1),FVC,FEV_(1)/FVC,PEF and relative expression of STAT-6 protein before and after 10 days of treatment,and improvement time of clinical symptoms were compared between the two groups.Results The total effective rate of the study group was higher than that of the control group(96.55%vs.86.21%,χ^(2)=3.940,P=0.047).After 10 days of treatment,FEV_(1),FVC,FEV_(1)/FVC,PEF of the two groups were higher than those before treatment,relative expression of STAT-6 protein was lower than that before treatment,and the increased/decreased range of the study group were greater than those of the control group(P<0.05 or P<0.01).Improvement time of cough,asthma,dyspnea in the study group were shorter than those of the control group(P<0.01).Conclusion Montelukast sodium combined with methylprednisolone have a good effect in the treatment of children with asthma,which can effectively improve the pulmonary function of children,and promote the clinical symptoms improved rapidly.
