达格列净治疗2型糖尿病的临床效果及对脂代谢和内脏脂肪的影响

摘　　要：目的观察达格列净治疗2型糖尿病的临床效果及对脂代谢和内脏脂肪的影响。方法选取2019年1月—2020年1月惠安县惠南华侨医院收治的2型糖尿病患者120例,依据随机数字表法分为达格列净组和二甲双胍组,每组60例。达格列净组给予达格列净片治疗,二甲双胍组给予盐酸二甲双胍片治疗,2组患者均治疗3个月。比较2组患者临床疗效,治疗前后空腹血糖、餐后2 h血糖、糖化血红蛋白、脂代谢指标、内脏相关血清指标[内脏脂肪素(visfatin)、内脏脂肪组织来源的丝氨酸蛋白酶抑制剂(vaspin)]及不良反应。结果治疗1个月及3个月后,达格列净组患者总有效率均高于二甲双胍组(χ^(2)=5.263,P=0.021;χ^(2)=5.175,P=0.022);2组患者空腹血糖、餐后2 h血糖水平及糖化血红蛋白均低于治疗前,且达格列净组低于二甲双胍组(P<0.05或P<0.01);2组患者三酰甘油(TG)、总胆固醇(TC)及低密度脂蛋白胆固醇(LDL-C)水平均低于治疗前,且达格列净组低于二甲双胍组(P<0.01);2组患者内脏脂肪指数、visfatin及vaspin水平均低于治疗前,且达格列净组低于二甲双胍组(P<0.01);2组患者均未出现明显药物不良反应。结论达格列净治疗2型糖尿病的临床效果较好,可改善患者脂代谢及内脏脂肪指标,具有较高的应用价值。Objective To observe the clinical effect of dagliazine in the treatment of type 2 diabetes mellitus and its effect on lipid metabolism and visceral fat.Methods A total of 120 patients with type 2 diabetes mellitus admitted to Huinan Overseas Chinese Hospital of Huian County from January 2019 to January 2020 were selected and divided into dagliazine group and metformin group according to random number table method,with 60 cases in each group.The dagliazine group was treated with dagliazine tablets,and the metformin group was treated with metformin hydrochloride tablets.Both groups were treated for 3 months.The clinical efficacy,fasting blood glucose,2 h postprandial blood glucose,glycosylated hemoglobin,lipid metabolism indexes,splanchnorelated serum indexes(visfatin and vaspin)before and after treatment and adverse reactions of the two groups were compared.Results After 1 and 3 months of treatment,the total effective rate of dagliazine group was higher than that of metformin group(χ^(2)=5.263,P=0.021;χ^(2)=5.175,P=0.022);Fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin levels in two groups were lower than before treatment,and those in the dagliazine group were lower than metformin group(P<0.05 or P<0.01);The levels of triacylglycerol(TG),total cholesterol(TC)and low density lipoprotein cholesterol(LDL-C)in two groups were lower than before treatment,and those in the dagliazine group were lower than metformin group(P<0.01);Visceral fat index,visfatin and vaspin levels in two groups were lower than before treatment,and those in the dagliazine group were lower than metformin group(P<0.01).There were no obvious adverse drug reaction in 2 groups.Conclusion The application effect of dapagliflozin in patients with type 2 diabetes mellitus is better,and it can significantly improve the lipid metabolism and visceral fat,so its application value in the patients with type 2 diabetes mellitus is higher.
