摘　　要：目的建立南京市中医院静脉用药调配中心(PIVAS)前置审方系统并对审方结果和干预情况进行分析,促进合理用药。方法采用该院合理用药系统及前置审方系统,统计2020年1—12月进入PIVAS的所有静脉输液医嘱,记录经前置审方系统拦截的不合理医嘱并进行分类统计分析。结果系统共审核静脉输液医嘱总数29745组,进入人工审核医嘱1625组,审核未通过1269组,退回修改医嘱589组,退回双签名680组,医嘱干预率为4.27%,医嘱干预成功率为46.41%。结论合理用药系统提供药品信息知识库,审方药师利用前置审方系统进行事前干预,可有效提高审方效率,提高医院合理用药水平,保证安全用药。Objective Establish the pre-description review system of pharmaceutical intravenous administration service(PIVAS)of Nanjing Traditional Chinese Medicine Hospital,and analyze the results of prescription review and intervention,in order to promote the rational use of drugs in hospitals.Methods The hospital′s rational drug use system and the pre-description review system were used to count all intravenous infusion orders entered into PIVAS from January to December 2020,recorded unreasonable orders intercepted by the pre-description review system,and carry out classification and statistical analysis.Results The total number of intravenous infusion orders reviewed by the system was 29745 groups,and there were 1625 groups of orders entered into manual review.There were 1269 groups that failed the review,589 groups returned the modified orders,and 680 groups that needed double signatures.The intervention rate of doctor′s order was 4.27%,and the success rate of doctor′s order intervention was 46.41%.Conclusion The rational drug use system provides a knowledge base of drug information,and the prescription review pharmacist uses the pre-description review system to conduct prior intervention,which can effectively improve the efficiency of prescription review,improve the level of rational drug use in the hospital,and ensure safe drug use.
