津沽脏腑推拿“得气”的中医内涵与应用思考

摘　　要：得气是针灸、推拿等治疗过程中患者最常见的表现。脏腑推拿是推拿的重要分支,津沽脏腑推拿则是脏腑推拿的代表性流派。除了操作方法与传统推拿有明显区别外,津沽脏腑推拿特别强调患者的得气、医者手下的感觉等,认为这是患者治疗效验的基础。通过介绍津沽脏腑推拿得气过程中畅通经脉、调理气血以达到治疗作用的理论基础,阐释津沽脏腑推拿得气过程中正邪演化的中医机理,总结患者常见的得气部位,分析患者凉、热、麻、酸、胀、潮(湿)、痛等得气表现的内在病机及其对疗效判定、疾病转归等方面的临床意义,为脏腑推拿的临床应用提供参考。Deqi(得气)is the most common manifestation of patients during acupuncture,moxibustion,tuina and other treatments.Zang-fu tuina is an important branch of tuina,and Jingu zang-fu tuina is a representative school.In addition to its obvious differences in operation methods with traditional tuina,Jingu zang-fu tuina especially emphasizes deqi of patients and the sensation of the physician's hand,which are considered to be the basis for the effectiveness of the treatment.This article focused on introducing the theoretical basis for the process of deqi in Jingu zangfu tuina by unblocking meridians and regulating qi and blood,explaining the internal mechanism of the evolution of both healthy and pathogenic qi in traditional Chinese medicine,describing the common sites of deqi in patients,re⁃vealing the internal pathogenesis of the manifestations of cold,heat,numbness,aching,distention,tidal(damp⁃ness),and pain in deqi and their clinical significance in determining the therapeutic effect and disease prognosis,so as to provides a reference for further understanding and application of zangfu tuina.
