摘 要:目的了解临床分离沙门菌对抗菌药物的耐药性及感染临床特征分析。方法2018—2021年从阜阳市人民医院住院患者临床标本分离菌株,经VITEK2compact仪器鉴定为沙门菌,用沙门菌诊断血清进行分型,采用纸片扩散法进行药敏试验,根据CLSI2022判读结果。结果从66571例患者的72784份标本中分离到82株沙门菌,以鼠伤寒血清型(27株,32.9%)、肠炎血清型(25株,30.5%)为主。59例肠道感染病例中发热55例,腹泻59例,腹痛17例,呕吐15例;23例侵袭性感染中21例发热,腹泻8例,腹痛7例,呕吐5例,肾周脓肿1例。分离到的沙门菌耐药率为:氨苄西林92.7%、氨苄西林/舒巴坦39.0%、头孢曲松14.6%、环丙沙星12.2%、复方新诺明35.4%、氯霉素34.1%,其中环丙沙星中介率为59.8%。结论我院临床分离沙门菌对多种抗生素耐药,氨苄西林不适用经验用药。Objective To analyze the clinical features and the antimicrobial resistance of Salmonella isolated from clinical samples. Methods The strains of Salmonella isolated from clinical samples of People's Hospital of Fuyang City were identified by VITEK2 compact instrument,serotyping was performed with Salmonella diagnostic serum.Antimicrobial susceptibility test was performed by Kirby-Bauer method and analyzed according to CLSI 2022.Results A total of 82 strains of Salmonella were isolated from 72 784 samples collected from 66 571 patients,which were mainly serotyped as Salmonella typhimurium(27 strains,32.9%) and Salmonella enteritidis(25 strains,30.5%).Among the 59 cases of intestinal infections,the numbers of patients with fever,diarrhea,abdominal pain and vomiting were 55,59,17 and 15 respectively. While the data were 21,8,7 and 5 among the 23 cases of invasive infections.In additional,1 case experienced perinephric abscess in invasive Salmonella infections. The resistance rates of Salmonella to ampicillin,ampicillin/sulbactam,ceftriaxone,ciprofloxacin,cotrimoxazole and chloramphenicol were 92.7%,39.0%,14.6%,12.2%,35.4% and 34.1% respectively,while the intermediate susceptibility rate to ciprofloxacin was59.8%. Conclusion The strains of Salmonella were severely resistant to multiple antibiotics. Ampicillin was not suitable for empiric use.
