白术健脾汤联合多潘立酮片治疗功能性消化不良脾虚气滞证45例临床观察

摘　　要：目的观察白术健脾汤联合多潘立酮片治疗功能性消化不良(FD)脾虚气滞证的临床疗效。方法将90例FD脾虚气滞证患者采用随机数字表法分为对照组和治疗组,各45例。对照组给予西药多潘立酮片口服,治疗组给予白术健脾汤联合多潘立酮片口服,治疗4周后比较2组的临床疗效,以及治疗前后的中医症状积分,血清5-羟色胺(5-HT)、神经肽Y(NPY)、胃动素(MTL)水平,唾液淀粉酶活性,D-木糖排泄率。结果对照组总有效率为75.56%(34/45),治疗组为93.33%(42/45),2组比较差异有统计学意义(P<0.05);治疗后2组主症各个症状积分及总积分明显降低,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组降低更明显,与对照组治疗后比较差异均有统计学意义(P<0.01);治疗后2组血清5-HT、NPY水平及唾液淀粉酶活性、D-木糖排泄率明显升高,治疗组血清MTL水平亦明显升高,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组升高更明显,与对照组治疗后比较差异均有统计学意义(P<0.05或P<0.01)。结论白术健脾汤联合多潘立酮片治疗FD脾虚气滞证患者疗效显著,可升高与胃肠功能相关的血清学指标水平,提高唾液淀粉酶活性、D-木糖排泄率,进而有效缓解患者的临床症状。Objective To observe the clinical efficacy of Baizhu Jianpi Tang(白术健脾汤)combined with Domperidone Tablets in the treatment of functional dyspepsia(FD)patients with spleen deficiency and qi stagnation syndrome.Methods 90 FD patients with spleen deficiency and qi stagnation syndrome were randomly divided into a control group and a treatment group by the random number table method,with 45 cases in each group.The control group was given oral administration of Domperidone Tablets,while the treatment group was given oral administration of Baizhu Jianpi Tang combined with Domperidone Tablets.After 4 weeks of treatment,the clinical efficacy of the two groups,the symptom integral of TCM,serum levels of 5-hydroxytryptamine(5-HT),neuropeptide Y(NPY),motilin(MTL),salivary amylase activity,and D-xylose excrtion rate before and after treatment were compared.Results The total effective rate of the control group was 75.56%(34/45),while that of the treatment group was 93.33%(42/45).The difference between the two groups was statistically significant(P<0.05).After treatment,the symptom integral of the main symptoms and total scores in the two groups significantly decreased,with statistical significance(P<0.05)compared to the same group before treatment,and the decrease was more significant in the treatment group,with statistical significance(P<0.01)compared to the control group after treatment.After treatment,the levels of serum 5-HT and NPY,salivary amylase activity,and D-xylose excretion rate significantly increased in the two groups,and the serum MTL levels in the treatment group were also significantly increased,with statistical significance(P<0.05)compared to the same group before treatment,and the increase in the treatment group was more significant,with statistical significance(P<0.05 or P<0.01)compared to the control group after treatment.Conclusion The combination of Baizhu Jianpi Tang and Domperidone Tablets has a significant therapeutic effect on FD patients with spleen deficiency and qi stagnation syndrome.It c
