摘　　要：目的拟比较心房颤动(AF)射频消融患者术后复发与非复发者左房(LA)容积、心房厚度和LA外脂肪体积差异.方法共纳入40例AF消融术后接受了计算机扫描成像血管造影(CTA)的患者(复发20例,非复发20例).通过3D-slicer图像软件将心脏CTA的LA容积、厚度和LA心外膜脂肪量定量,并比较两组患者的差异.结果两组间性别(男/女:12/8 vs 13/7,P>0.05)和年龄[(65.5±7.7)岁vs(66.7±6.7)岁,P>0.05]无显著差异.与非复发组相比,复发组LA体积更大[(154.5±43.6)ml vs(118±37.8)ml,P<0.01)],LA心外膜脂肪量更多[(16.9±6.3)g vs(12.3±5.8)g,P<0.05)].但两组间总心外脂肪量无显著差异[(128.6±58.3)g vs(124.9±63.2)g,P>0.05].另外,AF复发组左房壁各个方向上的厚度显著大于非复发组(P均<0.05).结论AF消融后复发与非复发相比,LA体积及心房外膜脂肪量更大,而心外总心外膜脂肪量无差异.Objective To compare the differences of left atrial(LA)volume,wall thickness and pericardial fat of LA between atrial fibrillation(AF)recurrence and non-recurrence in patients 12 months after radiofrequency ablation.Methods A total of 40 patients who underwent computed tomographicangiography(CTA)one year after ablation of AF(20 patients with recurrence and 20 without recurrence)were included in this retrospective analysis.The LA volume,wall thickness and LA epicardial fat mass of cardiac CTA were quantified by 3D-slicer image software.Results There were no significant differences in gender(male/female:12/8 vs 13/7,P>0.05)and age[(65.5±7.7)years vs(66.7±6.7)years,P>0.05]between the two groups.Compared with the non-recurrence group,the recurrence group after radiofrequency ablation had larger LA volume[(154.5±43.6)ml vs(118±37.8)ml,P<0.01],and greater left atrial epicardial fat mass[(16.9±6.3)g vs(12.3±5.8g),P<0.05].However,there was no significant difference in total pericardiac fat mass between the two groups[(128.6±58.3)g vs(124.9±63.2)g,P>0.05].Moreover,the left atrial wall thickness in the recurrent AF group was significantly greater than that in the non-recurrent group(all P values<0.05).Conclusion LA volume and atrial epicardial fat mass fat mass are larger in AF recurrent group after radiofrequency ablation.[Chinese Journal of Cardiac Pacing and Electrophysiology,2023,37(3):215-218]
