血清FT3、BNP水平在AECOPD患者中的变化及评估价值

摘　　要：目的观察慢性阻塞性肺疾病急性加重(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者血清游离三碘甲腺原氨酸(FT3)、脑钠肽(BNP)等指标的表达水平,比较各指标对患者疾病严重程度、预后的评估价值。方法回顾性分析304例AECOPD患者的临床资料,根据病情严重程度分为高危组(141例)和低危组(163例),根据预后分为死亡组(59例)和存活组(245例),分析各组血清FT3、BNP、降钙素原(PCT)、C-反应蛋白(CRP)的表达水平;绘制受试者工作特征曲线(ROC),分析各指标评估AECOPD患者疾病严重程度及预后的曲线下面积值(AUC)。结果高危组和低危组间,死亡组和存活组间,患者的血清FT3、BNP、PCT、CRP水平差异均有统计学意义(P<0.05)。FT3评估AECOPD患者疾病严重程度的AUC值(0.879)最高(Z=18.459,P<0.05),其敏感度和特异度分别为82.27%、83.44%;BNP评估AECOPD患者预后的AUC值(0.886)最高(Z=13.313、P<0.05),其敏感度和特异度分别为83.05%、90.20%。结论血清FT3和BNP表达水平与AECOPD患者病情进展关系密切,通过检测其水平可有效评估患者疾病严重程度及预后。Objective To observe the expression levels of serum-free triiodothyronine(FT3)and brain natriuretic peptide(BNP)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),and compare the evaluation value of different indicators for severity and prognosis of the disease.Methods The clinical data of 304 AECOPD patients were retrospectively analyzed and divided into a high-risk group(141 cases)and a low-risk group(163 cases)based on severity,as well as a death group(59 cases)and a survival group(245 cases)based on prognosis.The expression levels of serum FT3,BNP,procalcitonin(PCT),and C-reactive protein(CRP)in each group were analyzed.Receiver operating characteristic curve(ROC)was plotted,and the area under curve(AUC)was analyzed to evaluate the disease severity and prognosis of AECOPD patients.Results There were statistically significant differences in serum FT3,BNP,PCT,and CRP levels between the high-risk group and the low-risk group,between the death group and the survival group(P<0.05).The AUC value of FT3 in assessing the severity of AECOPD was found to be the highest(0.879)among all biomarkers tested(Z=18.459,P<0.05).Its sensitivity and specificity were calculated as 82.27%and 83.44%,respectively.The AUC of BNP for evaluating the prognosis of AECOPD was the largest(0.886)(Z=13.313,P<0.05).Its sensitivity and specificity were 83.05%and 90.20%.Conclusions The serum levels of FT3 and BNP are closely associated with the progression of AECOPD,serving as effective indicators for assessing disease severity and prognosis.
