摘 要:目的探讨二维斑点追踪分层应变技术对单纯2型糖尿病患者左室收缩功能的评估作用.方法选择60例单纯2型糖尿病患者作为试验组,另选择60名体检健康者作为对照组,采用二维斑点追踪分层应变技术评估两组研究对象的左室收缩功能.结果两组研究对象的左室心肌纵向应变峰值(GLS)及圆周应变峰值(GCS)自基底段至心尖段逐步增加,自内层至外层逐层递减(P<0.05);其中试验组的心尖段径向应变峰值(GRS)与基底段相比降低显著,且与对照组比较,试验组的心尖段径向应变峰值(GRS)较中间段以及基底段降低明显(P<0.05);试验组逐层逐段GLS、中间段GCS及心尖段GRS均显著低于对照组(P<0.05).结论在单纯2型糖尿病患者中,心肌损伤早在患者出现症状及射血分数降低之前就已发生,而二维斑点追踪分层应变技术可以更早期、更敏感地发现单纯2型糖尿病患者左室收缩功能的变化,为早期临床治疗提供依据,进而改善患者的生存质量.Objective To evaluate the left ventricular systolic function in simple type 2 diabetes patients by two dimensional speckle tracking layered strain technique.Method Sixty patients with simple type 2 diabetes were selected as the test group,and another 60 healthy people were selected as the control group.Two dimensional speckle tracking layered strain technology was used to evaluate the left ventricular myocardial contraction of the two groups,and the Results were compared and analyzed by measuring the peak strain of each myocardial layer and segment.Results The peak longitudinal strain(GLS)and peak circumferential strain(GCS)of the left ventricular myocardium in both study groups gradually increased from the basal segment to the apical segment,and decreased layer by layer from the inner layer to the outer layer(P<0.05);The peak radial strain(GRS)of the apical segment in the test group decreased significantly comparing to the basal segment,and the peak radial strain(GRS)of the apical segment in the control group decreased significantly comparing to the middle and basal segments(P<0.05);The GLS,middle segment GCS,and apical segment GRS of the test group were significantly lower than those of the control group(P<0.05).Conclusion In patients with simple type 2 diabetes,myocardial injury occurred long before the onset of symptoms and the decrease of ejection fraction.The two dimensional speckle tracking layered strain technology can detect the changes of left ventricular systolic function in patients with simple type 2 diabetes more early and sensitively,providing important help for clinical and early treatment,thus improving the quality of life of patients.
