摘　　要：目的观察老年评估联合集束化护理对肺癌患者晚期生活质量的影响。方法选取2020年10月至2022年10月的莆田学院附属医院放疗科的72例老年肺癌晚期患者作为研究对象,采用区组随机方法将纳入研究的患者分为A组(集束化护理组)和B组(常规护理组),各36例。两组均在入院时完成老年评估,A组采用集束化护理,B组采用常规护理。比较两组患者入院后2周的疼痛视觉模拟评分(VAS)、匹兹堡睡眠质量指数评分(PSQI)、焦虑自评量表评分(SAS)差异及不良护理事件发生率。结果A组患者入院2周后的VAS评分、PSQI评分、SAS评分均低于B组,差异有统计学意义(P<0.001),两组均未发生不良护理事件。结论老年评估联合集束化护理可以有效改善老年肺癌患者的疼痛、睡眠障碍及焦虑障碍,改善晚期生活质量。Objective To observe the effect of combined cluster care in geriatric assessment on the quality of life of patients with advanced lung cancer.Methods Seventy-two elderly patients with advanced lung cancer in the radiotherapy department of the Affiliated Hospital of Putian University from October 2020 to October 2022 were selected as the research objects.The patients were divided into group A(cluster nursing group)and group B(routine nursing group)by block randomized method,with 36 cases in each group.Both groups completed geriatric assessment on admission.Group A took cluster nursing,and group B took routine nursing.The visual analogue scale(VAS)score,Pittsburgh sleep quality index(PSQI)score,self-rating anxiety scale(SAS)score and incidence of adverse nursing events were compared between the two groups 2 weeks after admission.Results VAS score,PSQI score and SAS score of group A were lower than those of group B 2 weeks after admission,and the differences were statistically significant(P<0.001),and there were no adverse nursing events occurred in both groups.Conclusion Geriatric assessment combined with cluster care can effectively improve pain,sleep disorders and anxiety disorders in elderly patients with lung cancer,and improve the quality of life in late stage.
