摘  要:目的探讨血清长链非编码RNA(lncRNA)HOX转录反义基因间RNA(HOTAIR)、miR-206表达水平与急性呼吸窘迫综合征(ARDS)患者病情严重程度及预后的关系。方法选取110例ARDS患者,依据其入院时的氧合指数[PaO_(2)/吸入氧比例(FiO_(2))]分为ARDS轻度组37例、ARDS中度组41例、ARDS重度组32例,并根据入院28 d内的临床结局将其分为生存组74例和死亡组36例。比较不同病情严重程度及不同临床结局ARDS患者的一般资料及临床资料,血清HOTAIR、miR-206表达水平,采用COX回归模型分析ARDS患者入院28 d内死亡的影响因素,采用受试者工作特征(ROC)曲线分析血清HOTAIR和miR-206表达水平对ARDS患者入院28 d内死亡的预测效能,采用Pearson法对血清HOTAIR与miR-206表达水平进行相关性分析。结果ARDS轻度组、ARDS中度组、ARDS重度组患者的呼气末正压通气(PEEP)、急性生理与慢性健康评价Ⅱ(APACHEⅡ)评分及血清HOTAIR表达水平均依次升高,而血清miR-206表达水平依次降低(均P<0.05)。与生存组相比,死亡组患者的APACHEⅡ评分、PEEP及血清HOTAIR表达水平均更高,而PaO_(2)/FiO_(2)及血清miR-206表达水平均更低(均P<0.05)。COX回归分析结果显示,APACHEⅡ评分、PaO_(2)/FiO_(2)及血清HOTAIR表达水平、血清miR-206表达水平均是ARDS患者入院28 d内死亡的影响因素(均P<0.05)。ROC曲线分析结果显示,血清HOTAIR表达水平、血清miR-206表达水平及两者联合预测ARDS患者入院28 d内死亡的曲线下面积分别为0.845、0.837、0.943,特异度分别为87.80%、71.60%、89.20%,敏感度分别为75.00%、83.30%、91.70%。Pearson相关性分析结果显示,ARDS患者血清HOTAIR的表达水平与血清miR-206的表达水平呈负相关(P<0.05)。结论血清HOTAIR、miR-206的表达水平与ARDS患者的病情严重程度及预后密切相关,血清HOTAIR表达水平升高及血清miR-206表达水平降低均是ARDS患者入院28 d内死亡的危险因素,且ARDS患者血清HOTAIR�Objective To investigate the relation of serum long non-coding RNA(lncRNA)HOX transcript anti-sense intergenic RNA(HOTAIR)and miR-206 expressions with the severity and prognosis of patients with acute respiratory distress syndrome(ARDS).Methods A total of 110 ARDS patients were selected,and they were assigned to mild ARDS group(37 cases),moderate ARDS group(41 cases),or severe ARDS group(32 cases)according to oxygenation index(PaO_(2)/fraction of inspiration oxygen[FiO_(2)])at admission,and then patients were further assigned to survival group(74 cases)or death group(36 cases)according to the clinical outcomes within 28 days of admission.The general data and clinical data,and expressions of serum HOTAIRand miR-206 were compared between the ARDS patients with different severities and clinical outcomes.The influencing factors for ARDS patients′death within 28 days of admission were analyzed by using the COX regression model.The prediction efficiency of serum HOTAIRand miR-206 expressions on ARDS patients′death was analyzed by employing the receiver operating characteristic(ROC)curve.The correlation of serum HOTAIRexpression with serum miR-206 expression was analyzed by the application of the Pearson method.Results The positive end-expiratory pressure ventilation(PEEP),Acute Physiology and Chronic Health EvaluationⅡ(APACHEⅡ)score,and serum HOTAIRexpression of the mild ARDS group,the moderate ARDS group,and the severe ARDS group were elevated successively,whereas serum miR-206 expression was successively decreased in the groups as above(all P<0.05).Compared with the survival group,patients in the death group obtained higher APACHEⅡscore,PEEP,and serum HOTAIRexpression,while lower PaO_(2)/FiO_(2) and serum miR-206 expression(all P<0.05).The results of COX regression analysis revealed that APACHEⅡscore,PaO_(2)/FiO_(2),and serum HOTAIRand miR-206 expressions were the influencing factors for ARDS patients′death within 28 days of admission(all P<0.05).The results of ROC curve analysis revealed that the areas
