基于战略体验模块理论的ICU清醒患者住院体验的质性研究

摘　　要：目的了解清醒患者在ICU住院期间的入住体验,为提供更优质、更个性化的照护提供参考。方法采用便利抽样法,选取20例有ICU住院经历的患者,采用半结构式深入访谈法进行资料收集,并采用内容分析法对数据进行整理分析。结果按照战略体验模块理论将ICU清醒患者住院体验分为4个主题。(1)ICU清醒患者存在不良感官体验:疼痛、口渴、寒冷、环境吵闹、睡眠剥夺、活动受限。(2)复杂多样的情感体验:担心害怕、焦虑无助、想念亲属、后悔自责、牵挂外界、百无聊赖、经济压力、病情平稳后心情放松。(3)对医疗服务的感受以及对自身疾病的思考:对医务人员感激信任,对医务人员未能及时回应自身需求的抱怨与理解,认为将来难以恢复到以前的生活。(4)参与自身疾病的治疗与康复的行动体验:期望直接与医生沟通自身病情及参与决策,希望主动开展早期活动。结论ICU清醒患者的住院需求复杂多变,给ICU医护人员的治疗和护理带来挑战,未来研究还需要明确ICU清醒患者住院体验的特殊性,开发针对ICU清醒患者住院体验的评估工具,改善ICU患者的住院体验,为进一步提升医疗护理服务水平提供科学依据。Objective To understand the experience of conscious patients in intensive care unit(ICU),so as to provide a reference and basis for providing better and more personalized care.Methods Twenty patients with ICU hospitalization experience were selected using the convenience sampling method.They were given an in-depth interview using a semi-structured questionnaire.The data were sorted out and analyzed using content analysis method.Results According to the theory of strategic experience module,the hospitalization experience of sober patients in ICU was divided into 4 themes.The sensory experiences included pain,thirst,cold,sleep deprivation,noise in the environment and limited movement.The emotional experiences were consisted of fear,helplessness,missing relatives,remorse,worrying about the outside world,boredom,economic pressure,and relaxation.The psychological experiences covered gratitude and trust,complaining and understanding difficulty to return to their previous lives in the future.The action experience included communication,consultation and participation in decision-making,as well as early activities.Conclusion The complex and changeable hospitalization needs of ICU patients pose great challenges to ICU medical staff in their treatment and nursing.Future studies need to clarify the particularity of ICU patients,develop related evaluation tools and better their of ICU experience.
