摘　　要：目的探讨基于SHEL模型的专项管理对改善急诊ICU患者医疗器械相关压力性损伤(medical devicerelatedpressure injury,MDRPI)的效果。方法采用便利抽样法,选取长沙市某三级甲等医院急诊ICU收治的409例患者以及急诊ICU的54名护理人员为研究对象。按照入院顺序,将2021年6月-12月收治的198例患者设为对照组,将2022年1月-6月收治的的211例患者设为试验组。对照组采用常规护理,试验组在对照组的基础上采用基于SHEL模型的MDRPI预防专项管理。比较培训前后急诊ICU护士预防MDRPI知信行得分以及2组MDRPI发生率。结果培训后急诊ICU护士预防MDRPI知信行总分高于培训前(t=51.846,P<0.001),试验组MDRPI发生率低于对照组(χ^(2)=8.182,P=0.004)。结论在急诊ICU患者中实施基于SHEL模型的MDRPI预防专项管理,可以提高护士预防MDRPI知信行水平,降低患者MDRPI发生率,改善临床护理质量。Objective To explore the effect of special management based on SHEL model on relieving medical device related pressure injury(MDRPI)in emergency ICU.Methods Totally 409 patients treated in a grade-A tertiary hospital in Changsha and 54 nurses working in the emergency ICU were selected using the convenient sampling.A total of 198 patients admitted in the second half of year 2021 were selected into the control group,while 211 treated in the first half of 2022 were chosen into the experimental group.Both groups were given routine nursing,while the experimental group was additionally provided with the special MDRPI nursing based on the SHEL model.Then,the incidence of MDRPI,as well as the score of clinical nurses′MDRPI knowledge,belief and practice scale for prevention of critical patients,were compared.Results After the intervention,the total score of clinical nurses′MDRPI knowledge,belief and practice scale for prevention of critical patients improved significantly(t=51.846,P<0.001).Moreover,the MDRPI incidence of the experimental group was significantly lower than that of the control group(χ^(2)=8.182,P=0.004).Conclusion The implementation of special management based on SHEL model in MDRPI nursing of critical patients in emergency ICU can lower the incidence of MDRPI,improve the level of nurses′MDRPI knowledge,belief and practice,as well as promote the quality of MDRPI clinical nursing.
