无痛分娩中采取自由体位分娩对分娩结局的影响研究

摘　　要：目的 对无痛分娩产妇进行自由体位与常规卧位的干预,以探析可以缩减产程的有效措施,并分析其对分娩结局的影响。方法 选择医院产科2020年6月-2022年5月收治的64例初产妇,分为研究组(n=32)和对照组(n=32),研究组产妇麻醉后分娩时采取自由体位,对照组麻醉后分娩时采取常规仰卧位,观察对比两组产妇分娩结局、产程中干预情况、产程时间。结果 研究组剖宫产率低于对照组,自然分娩率则高于对照组,差异均有统计学意义(P<0.05);研究组人工破膜率等指标均显著更低(P<0.05);研究组三项产程时间指标均更短(P<0.05)。结论 对无痛分娩产妇进行自由体位干预,相较于常规仰卧位干预,能显著降低剖宫产概率,利于产妇自然分娩,在减少产程中人工干预的同时,还能缩减三项产程时间指标。Objective To explore effective measures to reduce labor process and analyze their effect on delivery outcomes by intervening in a free position and routine clinostatism for painless delivery mothers.Methods The paper chose 64 primiparous women admitted to obstetrics department of the hospital from June 2020 to May 2022,and divided them into study group(n=32)and control group(n=32).Study group was anesthetized and delivered in a free position,while control group was anesthetized and delivered in routine clinostatism.Delivery outcome,intervention during labor and labor duration was observed and compared between two groups.Results Cesarean section rate in study group was lower than control group,while natural delivery rate was higher than control group,with statistical significance(P<0.05).The artificial membrane breaking rate and other indicators in research group were significantly lower(P<0.05).Three indicators of labor process time in research group were all shorter(P<0.05).Conclusion Compared with regular clinostatism intervention,free body position intervention can significantly reduce incidence of cesarean section and facilitate natural delivery for painless delivery mothers,can reduce manual intervention during labor process,and three labor process time indicators.
