盐酸瑞芬太尼注射液用于老年病人普外科手术麻醉诱导及维持的临床分析

摘　　要：目的 结合仪征市人民医院普外科老年手术患者麻醉情况,分析瑞芬太尼临床麻醉的使用方法和效果。方法 本研究选取仪征市人民医院2018年1月-2021年7月收治的60例普外科老年手术患者为研究对象,并将随机分为观察组和对照组,每组30例。观察组采取瑞芬太尼麻醉诱导维持,对照组以常规麻醉维持为主。观察两组插管前后MAP、HR水平变化及术后恢复情况。结果 两组患者插管前后MAP变化对比结果显示,诱导前、诱导后及插管时MAP变化不明显,差异无统计学意义(P>0.05),拔管后MAP水平变化差异有统计学意义(P<0.05),且两组患者插管前后HR变化对比结果显示,诱导前、诱导后及插管时HR变化不明显,差异无统计学意义(P>0.05),拔管后HR水平变化差异有统计学意义(P<0.05),同时观察组患者呼吸恢复时间、苏醒时间及拔管时间好于对照组(P<0.05)。结论 普外科老年手术患者麻醉运用瑞芬太尼结合丙泊酚麻醉效果显著,可以提高患者麻醉水平,所以能够在患者麻醉中推广。Objective To analyze application and effect of remifentanil in clinical anesthesia in combination with anesthesia situation of elderly patients with general surgery in Yizheng City People's Hospital.Methods The study chose 60 elderly surgical patients admitted to Yizheng City People's Hospital from January 2018 to July 2021 as research objects,and divided them into observation group and control group randomly,with 30 cases in each group.Remifentanil anesthesia was induced and maintained in observation group,while routine anesthesia was mainly maintained in control group.Changes in MAP and HR levels before and after intubation and postoperative recovery in both groups were observed.Results Comparison of MAP changes before and after intubation between two groups showed,there was no significant change in MAP before induction,after induction and during intubation,with no statistically significant difference(P>0.05).Difference in MAP levels after extubation was statistically significant(P<0.05),comparison of HR changes before and after intubation between two groups showed,there was no significant change in HR before induction,after induction and during intubation,with no statistically significant difference(P>0.05).Difference in HR levels after extubation was statistically significant(P<0.05)observation group had better respiratory recovery time,recovery time and extubation time than control group(P<0.05).Conclusion Combination of remifentanil and propofol can achieve significant effect on anesthesia of elderly surgical patients,can improve anesthesia level,which is worthy of promotion in anesthesia of patients.
