摘　　要：目的 探讨针刺结合火龙罐治疗寒湿型腰椎间盘突出症的疗效及对血清炎症因子水平的影响。方法 120例寒湿型腰椎间盘突出症患者,根据治疗方法不同分为Ⅰ组和Ⅱ组,每组60例。Ⅰ组患者单纯实施针刺治疗,Ⅱ组患者实施针刺结合火龙罐治疗。比较两组患者临床疗效、不良反应发生情况及治疗前后日本骨科协会评估治疗(JOA)评分、视觉模拟评分法(VAS)评分、血清C反应蛋白(CRP)水平。结果 治疗14 d后,两组患者JOA评分均高于治疗前,VAS评分及血清CRP水平均低于治疗前,且观察组JOA评分(22.35±5.05)分明显高于对照组的(17.28±4.44)分,VAS评分(2.08±0.65)分、血清CRP水平(11.32±3.04)mg/L明显低于对照组的(2.65±0.68)分、(18.42±3.35)mg/L,差异有统计学意义(P<0.05)。观察组患者治疗总有效率为90%,高于对照组的75%,差异有统计学意义(P<0.05)。两组患者治疗过程中均未见明显不良反应。结论 针刺结合火龙罐治疗寒湿型腰椎间盘突出症疗效确切,能显著地减轻患者腰痛及改善腰椎功能障碍,还能更好地降低血清炎症因子水平,且安全性良好。Objective To discuss the effect of acupuncture combined with Huolong Cupping in the treatment of cold-dampness type of lumbar disc herniation.Methods A total of 120 patients with cold-dampness type of lumbar disc herniation were divided into groupⅠand groupⅡaccording to different treatment methods,with 60 cases in each group.GroupⅠwas treated with acupuncture alone,and groupⅡwas treated with acupuncture combined with Huolong Cupping.Both groups were compared in terms of clinical efficacy,occurrence of adverse reactions,Japanese Orthopaedic Association(JOA)score,visual analogue scale(VAS)score and serum C-reactive protein(CRP)level before and after treatment.Results After 14 d of treatment,JOA scores in the two groups were higher than those before treatment in this group,and VAS scores and serum CRP levels were lower than those before treatment in this group;JOA score of(22.35±5.05)points in the observation group was significantly higher than that of(17.28±4.44)points in the control group;VAS score of(2.08±0.65)points and serum CRP level of(11.32±3.04)mg/L in the observation group were significantly lower than those of(2.65±0.68)points and(18.42±3.35)mg/L in the control group;the differences were statistically significant(P<0.05).The total effective rate of the observation group was 90%,which was higher than that of 75%of the control group,and the difference was statistically significant(P<0.05).No obvious adverse reactions were observed in the two groups during treatment.Conclusion Acupuncture combined with Huolong Cupping in the treatment of cold-dampness type of lumbar disc herniation has a definite effect,which can significantly reduce the patients'lumbago and improve lumbar dysfunction,and can better reduce the level of serum inflammatory factors with high safety.
