芪蒌保元汤对于急性心肌梗死PPCI术后阳气亏虚证证候及康复的影响

摘　　要：目的:观察自拟芪蒌保元汤用于急性心肌梗死PPCI术后恢复期阳气亏虚证证候及术后康复的影响。方法 选取2018年10月—2022年1月于太和县中医院心血管科急诊入院的急性ST段抬高型心肌梗死(STEMI)并成功完成PPCI术患者,术后中医辨证属阳气亏虚者共46例,分为对照组(23例)和观察组(23例)。对照组给予常规药物治疗,观察组在常规治疗的基础上给予自拟芪蒌保元汤口服。观察2组6个月后的中医证候和症状,分析自拟中药汤剂对患者的影响。结果:2组患者治疗后观察组心脏LVEF高于对照组(P<0.05),治疗后2组患者心脏LVDD均有所增大,治疗后观察组心脏LVDD增大值低于对照组(P<0.05),治疗后2组患者6 min步行距离均较前增加,观察组增加值更明显(P<0.05),治疗后生活质量评分观察组高于对照组(P<0.05)。结论:自拟芪蒌保元汤在急性心肌梗死患者PPCI术成功后应用具有安全性,可以抑制心脏重构,能够改善临床症状,提高生活质量,从而促进术后康复。Objective:To observe the effect of self-made Qilou Baoyuan decoction on the syndrome of Yang Qi deficiency and postoperative rehabilitation in the convalescent period of acute myocardial infarction after PPCI.Methods Patients with acute ST-segment elevation myocardial infarction(STEMI)who were admitted to the emergency department of Cardiovascular Department of Taihe County Hospital of Traditional Chinese Medicine from October 2018 to January 2022 were selected and successfully completed PPCI.A total of 46 patients were divided into control group(23 cases)and observation group(23 cases).The control group was given conventional drug treatment,and the observation group was given oral Qilou Baoyuan decoction on the basis of conventional treatment.Observe the TCM syndromes and symptoms of the two groups after 6 months,and analyze the influence of the self-made TCM decoction on the patients.Results:After treatment,the heart LVEF of the observation group was higher than that of the control group(P<0.05).The 6-min walking distance of patients in both groups increased after treatment,and the increase was more obvious in the observation group(P<0.05).After treatment,the quality of life score in the observation group was higher than that in the control group(P<0.05).Conclusion:Self-made Qilou Baoyuan decoction is safe for patients with acute myocardial infarction after successful PPCI operation.It can inhibit cardiac remodeling,improve clinical symptoms,improve quality of life,and promote postoperative recovery.
