金鼎汤联合地西泮对乳腺癌术后心胆气虚型失眠症的影响

摘　　要：目的:探究清代名医黄元御《四圣心源》中所创金鼎汤联合地西泮对乳腺癌术后心胆气虚型失眠症的影响。方法:选择2019年10月—2021年8月山东省潍坊市中医院就诊的乳腺癌术后失眠中医证候为心胆气虚型患者90例,按随机数字表法分为对照组和实验组,每组45例,2组均给予口服地西泮治疗,实验组加用金鼎汤治疗,疗程8周。疗效采用有效率、汉密尔顿焦虑量表(Hamilton Anxiety Scale,HAMA)评分、匹兹堡睡眠质量指数量表(Pittsburgh sleep quality index,PSQI)评分、血清白介素6(IL-6)和肿瘤坏死因子-α(TNF-α)进行评价,并观察患者的复发或加重比例。结果:对照组有效率为77.78%,实验组为95.56%,2组有效率差别具有统计学差异;相比对照组,实验组能够明显改善患者的HAMA和PSQI评分;实验组能够明显改善患者血清IL-6(P<0.05)和TNF-α水平(P<0.01);对照组失眠复发/加重比例为17.78%,实验组为2.22%,差别均具有统计学差异。结论:金鼎汤联合地西泮能够明显减轻乳腺癌术后心胆气虚型失眠患者的焦虑情绪,改善其睡眠情况。Objective:To explore the effect of Jinding decoction combined with diazepam created in“Sisheng Xinyuan”by HUANG Yuanyu,a famous doctor in Qing Dynasty,on insomnia of heart-gallbladder-qi deficiency type after breast cancer surgery.Methods:Select 90 cases of insomnia after breast cancer surgery in Weifang Hospital of Traditional Chinese Medicine,Shandong Province from October 2019 to August 2021.There were 45 cases in the first group,both groups were given oral diazepam,and the experimental group was additionally treated with Jinding Decoction,the course of treatment was 8 weeks.Effective rate,Hamilton Anxiety Scale(HAMA)score,Pittsburgh sleep quality index(PSQI)score,serum interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)to evaluate and observe the recurrence or exacerbation ratio of patients.Results:The effective rate of the control group was 77.78%,and that of the experimental group was 95.56%.The difference in effective rate between the two groups was statistically significant;compared with the control group,the experimental group could significantly improve the patient’s HAMA and PSQI scores;the experimental group could significantly improve the serum IL-6(P<0.05)and TNF-αlevels(P<0.01);the rate of recurrence/exacerbation of insomnia in the control group was 17.78%,and that in the experimental group was 2.22%,the differences were statistically significant.Conclusion:Jinding decoction combined with diazepam can significantly reduce the anxiety and improve sleep in patients with insomnia due to heart and gallbladder Qi deficiency after breast cancer surgery.
