摘  要:目的:采用Meta分析的方法,比较单孔与多孔腹腔镜肝切除术的安全性、有效性。方法:检索PubMed、Embase、Cochrane Library、中国知网、万方等数据库已发表的有关单孔与多孔腹腔镜肝切除术近期效果对比的文献资料。对符合条件的数据进行Meta分析,采用RevMan 5.3软件进行统计处理。结果:符合纳入标准的文献共10篇,包括807例患者,其中单孔组361例,多孔组446例。单孔组术中出血量、术后住院时间少于多孔组,差异有统计学意义(P<0.05);两组手术时间、中转开腹率、手术切缘及术后并发症差异无统计学意义(P>0.05)。结论:单孔腹腔镜肝切除术的安全性基本与多孔腹腔镜手术一致,更利于患者的早期恢复,但目前仍需选择适当的病例谨慎开展。Objective:To compare the safety and effectiveness of single-port with multi-port laparoscopic hepatectomy.Methods:PubMed,Embase,Cochrane Library,CNKI and Wanfang databases were searched for published clinical literatures on comparative studies of short-term results between single-port and multi-port laparoscopic hepatectomy.Meta-analysis was performed for all eligible results u sing RevMan 5.3 statistical software.Results:Ten literatures consisting of 807 patients were included in this study.A total of 361 p atients received single-port laparoscopic hepatectomy and 446 patients underwent multi-port laparoscopic hepatectomy.Compared with the multi-port group,single-port group was associated with significantly fewer blood loss and shorter postoperative hospital stay.In terms of operation time,rate of conversion to laparotomy,incisional margin and postoperative complications,there were no significant diffe-rences between the two groups.Conclusions:Compared with multi-port laparoscopic surgery,the single-port laparoscopic hepatectomy shows comparable safety and better efficacy of recovery.However,it is still necessary to select appropriate cases and proceed cautiously.
