脾肾双补方治疗脾肾两虚型特发性矮小症临床观察

摘　　要：目的 探讨脾肾双补方对脾肾两虚型特发性矮小症(Idiopathic short stature, ISS)患儿的临床疗效及对GH-IGF-1轴的影响。方法 收集2020年7月—2022年1月于天津中医药大学第二附属医院儿科门诊就诊的脾肾两虚型ISS患儿34例,运用随机数字表法随机分为对照组19例、观察组15例。对照组予重组人生长激素皮下注射,观察组予科内协定处方脾肾双补方颗粒剂冲服,同时2组均予生活方式指导,比较治疗前、治疗6个月后身高、身高半年生长速度总有效率、血清IGF-1水平、中医证候积分、试验期间用药总花费。结果 脾肾双补方与rhGH均能改善患儿身高、身高半年生长速度总有效率及血清IGF-1水平,中医在治疗价格方面具有明显优势。结论 脾肾双补方可升高ISS患儿的血清中IGF-1的浓度,从而提高ISS患儿的身高,提示脾肾双补方促生长作用机制可能跟上调血清IGF-1水平、调节GH-IGF-1生长轴功能有关,为本试验的创新点。Objective To explore the clinical effect of tonifying spleen and kidney recipe in the treatment of idiopathic short stature(ISS)of deficiency of spleen and kidney type.Methods From July 2020 to January 2022,34 children with ISS of deficiency of spleen and kidney type were collected from the pediatric clinic of the Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine.They were randomly divided into control group(19 cases)and observation group(15 cases)by random number table.The control group was injected with recombinant human growth hormone subcutaneously,and the observation group took tonifying spleen and kidney recipe.At the same time,the two groups were given life-style guidance.The height,the growth rate and the total effective rate in half a year,the serum ICF-1 level,the traditional Chinese medicine syndrome score and the total drug cost during the trial were compared before and after the treatment for 6 months.Results Tonifying spleen and kidney recipe and rhH can improve the height,total effective rate of growth rate in half a year and serum ICF-1 level of infants with serum IGF-1 level,and traditional Chinese medicine has obvious advantages in the treatment price.Conclusio Tonifying spleen and kidney recipe can increase the serum ICF-1 concentration in ISS children,thereby improving the height of ISS children,suggesting that the mechanism of spleen and kidney double supplement may be related to up-regulation of serum ICF-1 level and regulation of GH-IGF-1 axis function,which is the innovation of this experiment.
