基于“脑脾同治法”论治紧张型头痛经验介绍

摘　　要：头痛病主要责之于风、火、痰、瘀、虚,而致病关键在于瘀。本文基于脾与脑经络相通,治头痛当以脾胃为中心;脾主升清,清阳得养则脑清神聪;脾主运化,气血津液条达则脑窍荣畅;脾主统血,脉行得道可奉养脑窍等四个方面,倡导“脑脾同治法”论治紧张型头痛。常用治法有:醒脾行气开郁,活血化瘀开窍;运脾理气化痰,活血化瘀通窍;健脾益气填髓,活血化瘀益窍。以健脾祛瘀自拟方为基础方,随症加减,气滞血瘀者,合血府逐瘀汤加减;痰瘀阻络者,合化痰通络汤加减;血虚血瘀者,合补阳还五汤加减。The main responsibility for headache disease is Wind,Fire,Phlegm,Stasis,and Deficiency,and the key lies in Stasis.This article is based on four aspects including that Spleen and brain meridians are connected,causing that the treatment is centered on Spleen and Stomach;Spleen is in charge of transporting nutrients upwards,Qingyang is nourished,and brain is clear;Spleen controlls transportation and transformation,ensuring Qi-blood-body fluid run smoothly,and brain is prosperous;Spleen is responsible for keeping Blood within the vessels,accordingly the pulse is enlightened,and brain is nourished.Consequently,“Joint treatment of brain and Spleen”should be advocated.Common treatments include:wake up Spleen and promote circulation of the Qi to expel stagnation,activate Blood and dissolve Stasis to open the brain-stasis;transport Spleen and regulating Qi to vaporize phlegm,activate Blood and dissolve Stasis to dredge brain-stasis;tonify Spleen and Qi to fill the marrow,activate Blood and dissolve Stasis to nourish Qi-blood-body fluid.Based on the self-proposed prescription of invigorating Spleen and removing Blood Stasis,the addition and subtraction of symptoms such as Xuefuzhuyu decoction when facing Qi stagnation and Blood Stasis,Huatantongluo decoction when facing Phlegm Blood Stasis obstruction,Buyanghuanwu decoction when facing Blood deficiency and Blood Stasis.
