引流挂线联合加味苦参汤坐浴对高位复杂性肛瘘患者的临床疗效分析

摘　　要：目的:分析高位复杂性肛瘘应用引流挂线联合加味苦参汤坐浴治疗的效果,以及对患者疼痛、创面渗液及愈合时间的影响。方法:选取2020年1月—2021年5月长垣市中医医院收治的128例高位复杂性肛瘘患者作为研究对象,分析其临床病历资料,根据治疗方案的不同,将患者分为对照组和观察组,每组各64例。对照组采用引流挂线联合高锰酸钾溶液熏洗坐浴治疗;观察组采用引流挂线联合加味苦参汤熏洗坐浴治疗,对比两种治疗方案的差异性(临床疗效、术后疼痛评分、创面渗液及肉芽形态评分、上皮生长、脓苔脱落及创面愈合时间等)。结果:观察组治疗总有效率高于对照组,差异有统计学意义(χ^(2)=7.821,P<0.05);观察组视觉模拟评分法(VAS)及肛门功能评分均低于对照组,差异有统计学意义(t=13.102、7.360,P<0.05);术后7 d,观察组创面渗液及肉芽形态评分低于对照组,差异有统计学意义(t=3.726、6.000,P<0.05);观察组上皮生长时间、脓苔脱落时间、创面愈合时间、VAS评分和肛门功能评分均低于对照组,差异有统计学意义(t=14.370、14.366、22.450,P<0.05)。结论:高位复杂性肛瘘患者经引流挂线和加味苦参汤坐浴联合治疗,可减轻疼痛,促进脓苔脱落与创面愈合,疗效显著。Objective:To analyze the effect of applying drainage and hanging sutures combined with sitz bath with added bitter ginseng soup in the treatment of high-grade complex anal fistula,and the effect on patients’pain,wound exudate and healing time.Methods:128 patients with high complex anal fistula admitted to the hospital from January 2020 to May 2021 were selected as study subjects,and their clinical history data were analyzed according to the treatment plan,and the patients were divided into a control group and an observation group,with 64 cases in each group.The control group was treated with drainage and hanging lines combined with potassium permanganate solution fumigation and sitz bath,while the observation group was treated with drainage and hanging lines combined with fumigation and sitz bath with added bitter ginseng soup.The differences between the two treatment regimens were compared(clinical efficacy,postoperative pain score,wound exudate and granulation morphology score,epithelial growth,pus moss shedding,and wound healing time).Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(χ^(2)=7.821,P<0.05).The VAS and anal function scores in the observation group were lower than those in the control group,with statistically significant differences(t=13.102,7.360,P<0.05).At 7 d postoperatively,the wound exudate and granulation morphology scores of the observation group were lower than those of the control group,with statistically significant differences(t=3.726,6.000,P<0.05).The epithelial growth time,pus moss shedding time,wound healing time,VAS score and anal function score were lower in the observation group than in the control group,and the differences were statistically significant(t=14.370,14.366,22.450,P<0.05).Conclusion:The combined treatment of drainage and ligature and sitz bath with added bitter ginseng soup in patients with high complex anal fistula can reduce pain and promote pus moss she
