腹腔镜肾囊肿去顶术治疗单纯性肾囊肿的效果及对患者手术应激指标水平的影响

摘　　要：目的:探讨腹腔镜肾囊肿去顶术治疗单纯性肾囊肿的效果及对患者血清皮质醇(Cor)、肾上腺素(AD)、去甲肾上腺素(NE)、血管紧张素Ⅱ(Ang-Ⅱ)水平的影响。方法:选取2019年1月—2020年1月佳木斯市中心医院收治的70例单纯性肾囊肿患者作为研究对象。对照组(n=35)采用开放手术治疗,观察组(n=35)采用腹腔镜肾囊肿去顶术治疗。比较两组患者围术期情况、手术应激指标、肾功能指标、并发症发生情况。结果:观察组术中出血量、下床活动时间、术后住院天数少于对照组,差异有统计学意义(t=29.631、23.902、13.406,P<0.05);术后12 h,两组患者血清Cor、AD、NE、Ang-Ⅱ水平均比术前升高,但观察组低于对照组,差异有统计学意义(t=6.092、13.127、5.086、6.802,P<0.05);术后12 h,两组患者血清血肌酐(SCr)、尿素氮(BUN)和尿红细胞(RBC)、尿白细胞(WBC)水平比术前升高,但观察组低于对照组,差异有统计学意义(t=7.202、8.592、6.799、11.800,P<0.05);观察组并发症发生率明显低于对照组,差异有统计学意义(χ^(2)=4.629,P<0.05)。结论:腹腔镜肾囊肿去顶术治疗单纯性肾囊肿具有出血少、术后恢复快、手术应激小、对肾功能影响小、并发症少的优势。Objective:To investigate the effect of laparoscopic renal cyst decortication for simple renal cysts and the effect on serum cortisol(Cor),epinephrine(AD),norepinephrine(NE)and angiotensin II(Ang-II)levels in patients.Methods:70 patients with simple renal cysts admitted to the hospital from January 2019 to January 2020 were selected as the study population.Open surgery was used in the control group(n=35),and laparoscopic renal cyst debulking was used in the observation group(n=35).The perioperative conditions,surgical stress indicators,renal function indicators,and complications were compared between the two groups.Results:The amount of bleeding,the time to get out of bed and the number of postoperative hospital days in the observation group were less than those in the control group,and the differences were statistically significant(t=29.631,23.902,13.406,P<0.05).At 12 h after surgery,serum Cor,AD,NE,and Ang-II levels were higher in both groups than before surgery,but the observation group was lower than the control group,and the difference was statistically significant(t=6.092,13.127,5.086,6.802,P<0.05).At 12 h after surgery,serum SCr,BUN and urinary RBC and urinary WBC were higher in both groups than before surgery,and the observation group was lower than the control group,and the difference was statistically significant(t=7.202,8.592,6.799,11.800,P<0.05).The incidence of complications was significantly lower in the observation group than in the control group,and the difference was statistically significant(χ^(2)=4.629,P<0.05).Conclusion:Laparoscopic renal cyst decortication for simple renal cysts has the advantages of less bleeding,faster postoperative recovery,less surgical stress,less impact on renal function and fewer complications.
