小牛血去蛋白提取物眼用凝胶与重组牛碱性成纤维细胞生长因子凝胶在角膜损伤修复中的临床效果

摘　　要：目的:比较小牛血去蛋白提取物眼用凝胶与重组牛碱性成纤维细胞生长因子凝胶在角膜损伤修复中的临床效果,为验证临床治疗的实用性与可靠性提供依据。方法:回顾性分析2020年1月—2021年5月信阳市眼科医院收治的230例角膜损伤患者的一般资料,根据不同治疗方案将患者分为研究组(n=115)和对照组(n=115),对照组给予重组牛碱性成纤维细胞生长因子凝胶,研究组给予小牛血去蛋白提取物眼用凝胶,疗程均为14 d。对比两组患者治疗前后不同时间的眼部功能指标[泪液分泌试验(Schirmer test,SIt)、角膜荧光染色(corneal fluorescein staining,CFS)、泪膜破裂时间(tear break-up time,BUT)]、临床症状评分[眼表疾病指数量表(ocular surface disease index,OSDI)]及不良反应发生情况。结果:治疗后研究组SIt高于对照组,CFS低于对照组,差异有统计学意义(t=2.743、2.079,P<0.05);治疗7 d、14 d、30 d研究组OSDI均低于对照组,差异有统计学意义(t=3.389、5.042、4.651,P<0.05);两组患者总不良反应率研究组为5.22%,略高于对照组为4.35%,两组患者不良反应比较,差异无统计学意义(χ^(2)=0.096,P>0.05)。结论:小牛血去蛋白提取物眼用凝胶与重组牛碱性成纤维细胞生长因子凝胶治疗角膜损伤的疗效显著且安全性均较高,但小牛血去蛋白提取物眼用凝胶疗效更佳,可有效改善患者眼部功能,降低临床症状评分。Objective:To compare the clinical effects of calf blood deproteinized extract ophthalmic gel with recombinant bovine basic fibroblast growth factor gel in corneal injury repair to provide a basis for verifying the usefulness and reliability of clinical treatment.Methods:The general data of 230 patients with corneal injury admitted to Xinyang Eye Hospital from January 2020 to May 2021 were retrospectively analyzed,and the patients were divided into a study group(n=115)and a control group(n=115)according to different treatment protocols.The control group was given recombinant bovine basic fibroblast growth factor gel,and the study group was given calf blood deproteinized extract ophthalmic gel,and the duration of treatment was 14 d for both groups.The ocular function indices(Schirmer test[SIt],corneal fluorescein staining[CFS],tear film break-up time[BUT]),clinical symptom scores(ocular surface disease index scale[OSDI])and adverse effects were compared between the two groups of patients before and after treatment at different times.Results:After treatment,SIt was higher in the study group than in the control group,and CFS was lower than in the control group,with statistically significant differences(t=2.743,2.079,P<0.05).The OSDI was lower in the study group than in the control group at 7 d,14 d,and 30 d of treatment,with statistically significant differences(t=3.389,5.042,4.651,P<0.05).The overall adverse reaction rate in the study group was 5.22%,which was higher than 4.35%in the control group,and the difference was not statistically significant(χ^(2)=0.096,P>0.05).Conclusion:Both calf blood deprotein extract ophthalmic gel and recombinant bovine basic fibroblast growth factor gel were effective and safe in the treatment of corneal injury,but calf blood deprotein extract ophthalmic gel was more effective in improving ocular function and reducing clinical symptom scores.
