基于炎性因子水平变化和皮损改善探讨苍肤止痒酊治疗结节性湿疹的应用价值

摘　　要：目的探讨苍肤止痒酊治疗对结节性湿疹患者炎性因子水平变化和皮损改善情况的影响。方法选取2021年1月-2022年2月在本院治疗的180例结节性湿疹患者,按随机数字表法分为A、B、C 3组,各60例。A组采用苍肤止痒酊治疗,B组采用卤米松乳膏治疗,C组采用酒精治疗。3组均治疗35 d。比较3组治疗35 d后的临床疗效,治疗前、治疗35 d后的炎性因子、皮损改善情况、特应性皮炎积分(SCORAD)、严重程度指数评分(EASI)、生活质量,治疗期间的安全性。结果A组治疗35 d后的总有效率为93.33%,高于B组(80.00%)及C组(63.33%),B组高于C组(P<0.05)。治疗35 d后,3组血清白介素6(IL-6)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)、白三烯B4(LTB4)水平、面积、色泽、丘疹、瘙痒、SCORAD、EASI、皮肤病生活质量评分表(DLQI)评分均比治疗前低,且与C组、B组比较,A组较低,与C组比较,B组较低;3组血清干扰素γ(IFN-γ)水平均比治疗前高,且与C组、B组比较,A组较高,与C组比较,B组较高(P<0.05)。治疗期间,A组、B组、C组不良反应发生率分别为5.00%、6.67%、13.33%,但3组间比较,差异无统计学意义(P>0.05)。结论苍肤止痒酊治疗结节性湿疹可有效缓解患者皮损症状,减轻患者炎症反应,进而有效提高治疗效果,促进患者生活质量的改善,且安全性良好。Objective To investigate the effect of Cangfu Zhiyang Tincture on the changes of inflammatory factor levels and the improvement of skin lesions in patients with nodular eczema.Methods 180 patients with nodular eczema who were treated in Qinhuangdao Hospital of Traditional Chinese Medicine from January 2021 to February 2022 were divided into three groups,namely group A,group B,and group C according to random number table method,with 60 patients in each group.Group A was treated with Cangfu Zhiyang Tincture,group B was treated with halometasone cream,and group C was treated with alcohol.All three groups were treated for 35 days.The clinical efficacies of the three groups after 35 days of treatment were compared.In the three groups inflammatory factors,improvement of skin lesion,SCORing atopic dermatitis(SCORAD)score,eczema area and severity index(EASI)score,quality of life before treatment and after 35 days of treatment,and safety during treatment were compared.Results After 35 days of treatment,the total effective rate in group A was 93.33%,which was higher than that in group B(80.00%)and group C(63.33%),and group B was higher than group C(P<0.05).After 35 days of treatment,the levels of serum interleukin-6(IL-6),interleukin-4(IL-4)and tumor necrosis factor-α(TNF-α),leukotriene B4(LTB4),and scores of area,color,papule,pruritus,SCORAD,EASI and dermatology quality of life(DLQI)of the three groups were lower than those before treatment,and compared with group C and group B,group A was lower;compared with group C,group B was lower.The levels of serum interferonγ(IFN-γ)of the three groups were higher than those before treatment,and compared with group C and group B,group A was higher;and compared with group C,group B was higher(P<0.05).During the treatment,the incidences of adverse reactions in group A,group B and group C were 5.00%,6.67%and 13.33%respectively,but there was no statistically significant difference among the three groups(P>0.05).Conclusion Cangfu Zhiyang Tincture can effectively relieve the symptoms
