摘 要:目的检测Linc00511在肺癌患者血清中的表达水平,探讨其单独检测及与CYFRA联合检测在肺癌诊断中的临床价值。方法选取2017年9月—2018年2月在中国医学科学院肿瘤医院就诊的60例肺癌患者及同期来院体检的79名健康人为研究对象,实时荧光定量PCR技术检测肺癌患者和健康对照者血清中Linc00511的表达水平,通过受试者工作特征(ROC)曲线、曲线下面积(AUC)、危险分数分析评估Linc00511对肺癌的诊断效能,计算灵敏度、特异度、阳性预测值(PPV)和阴性预测值(NPV),探讨独立及联合检测在肺癌诊断中的应用价值。结果对照组和肺癌组血清Linc00511的相对表达量分别为0.787(0.397,1.643)和1.933(1.315,3.229),二者比较差异有统计学意义(Z=-5.939,P<0.001);远处转移程度不同的患者组间比较,差异有统计学意义(Z=-2.226,P<0.05)。Linc00511独立检测诊断肺癌的AUC为0.806,敏感度、特异度、PPV、NPV分别为98.3%、25.3%、50.0%和95.2%,CYFRA诊断肺癌的AUC为0.704,敏感度、特异度、PPV和NPV分别为41.7%、82.3%、64.1%和65.0%,二者联合诊断的AUC为0.859,PPV、NPV分别提高为78.0%和71.4%。结论Linc00511在肺癌血清中高表达,是肺癌临床筛查的潜在血清学标志物。Objective To detect the expression level of Linc00511 in the serum of patients with lung cancer,and to explore the clinical value of LinC00511 alone and combined with CYFRA in the diagnosis of lung cancer.Methods A total of 60 patients with lung cancer and 79 healthy people who came to the Cancer Hospital of Chinese Academy of Medical Sciences from September 2017 to February 2018 were selected as the research objects.The expression levels of Linc00511 in serum of lung cancer patients and healthy controls were detected by real-time fluorescence quantitative PCR.The diagnostic efficacy of Linc00511 for lung cancer was evaluated by receiver operating characteristic(ROC)curve,area under the curve(AUC)and risk score analysis.The sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)were calculated.To explore the application value of independent and combined detection in the diagnosis of lung cancer.Results The relative expression levels of serum Linc00511 in the control group and the lung cancer group were 0.787(0.397,1.643)and 1.933(1.315,3.229),respectively,the difference was statistically significant(Z=-5.939,P<0.01).There was statistically significant difference between the patients with different degrees of distant metastasis(Z=-2.226,P<0.05).The AUC of Linc00511 in the diagnosis of lung cancer was 0.806,the sensitivity,specificity,PPV and NPV were 98.3%,25.3%,50.0%and 95.2%,respectively.The AUC of CYFRA in the diagnosis of lung cancer was 0.704,the sensitivity,specificity,PPV and NPV were 41.7%,82.3%,64.1%and 65.0%,respectively.The AUC of the combined diagnosis was 0.859,and the PPV and NPV were increased to 78.0%and 71.4%,respectively.Conclusion Linc00511 is highly expressed in serum of lung cancer and is a potential serological marker for clinical screening of lung cancer.
