慢性肾脏病患者别嘌醇超敏综合征15例分析

摘　　要：目的分析慢性肾脏病患者别嘌醇超敏综合征发生的特点,提高对其认识。方法回顾性分析近10年诊治的15例慢性肾脏病合并别嘌醇超敏综合征患者。结果男8例,女7例,潜伏期7~50 d。以皮疹伴/不伴发热为首发症状,肝肾损害发生率100%,9例尿毒症。其次是血液系统受累、免疫球蛋白E(IgE)水平升高。予甲基强的松龙1~2 mg/(kg·d)抗炎,部分患者联合透析、血液灌流、丙种球蛋白治疗。住院时间9~150 d。3例患者未能摆脱透析,1例患者死亡。结论应重视别嘌醇超敏综合征的危害性,严格把握慢性肾脏病患者降尿酸药物使用指征。Objective To analyze the characteristics of allopurinol hypersensitivity syndrome in chronic kidney disease and improve the recognition of its particularity.Methods The clinical data of 15 patients were retrospectively analyzed and summarized.Results 7 cases were male.The incubation period was 7~50 days.All cases were characterized by skin lesions and damage of liver and kidney function.9 cases developed uremia.Blood system involvement was followed.Methylprednisolone 1~2 mg/kg·d was given to the patients,and some patients were treated with combined blood purification and gamma globulin.Three patients failed to get off dialysis and one patient died.Conclusion The allopurinol medication should be strict in patients with chronic kidney disease.
