腹腔镜下全子宫切除术与经腹全子宫切除术的临床效果比较

摘　　要：目的对比腹腔镜下全子宫切除术、经腹全子宫切除术在临床中的治疗效果。方法抽取30例接受全子宫切除术患者为研究对象,根据手术方法不同分为对照组和治疗组,每组15例。对照组使用经腹全子宫切除术,治疗组使用腹腔镜下全子宫切除术。比较两组治疗效果。结果手术前,两组白细胞计数、C-反应蛋白水平比较,差异无统计学意义(P>0.05);手术后,两组白细胞计数、C-反应蛋白水平均有所升高,但治疗组白细胞计数(10.09±0.33)×10^(9)/L、C-反应蛋白(11.84±1.65)mg/L均低于对照组(11.75±1.64)×10^(9)/L、(30.21±1.79)mg/L(P<0.05)。手术后,治疗组生理功能、精神状态及总体健康评分高于对照组(P<0.05)。治疗组并发症发生率低于对照组,但对比差异无统计学意义(P>0.05)。治疗组术中出血量少于对照组,手术总时长、术后首次排气时间短于对照组(P<0.05)。结论与经腹全子宫切除术的临床疗效相比,腹腔镜下全子宫切除术的总体疗效更加突出,各项指标均显著更优,适合普及使用。Objective To compare the clinical therapeutic effect of laparoscopic total hysterectomy and transabdominal total hysterectomy.Methods 30 patients who underwent total hysterectomy were selected as the study objects and divided into control group and treatment group according to different surgical methods,with 15 cases in each group.The control group received total abdominal hysterectomy,and the treatment group received total laparoscopic hysterectomy.The therapeutic effect of the two groups was compared.Results After the operation,the white blood cell count and C-reactive protein were increased.However,the leukocyte count(10.09±0.33)×10^(9)/L and C-reactive protein(11.84±1.65)mg/L in the treatment group were lower than those in the control group(11.75±1.64)×10^(9)/L and(30.21±1.79)mg/L(P<0.05).After operation,the physiological function,mental state and overall health scores of the treatment group were higher than those of the control group(P<0.05).The incidence of complications in the treatment group was lower than that in the control group,but there was no significant difference between the two groups(P>0.05).The amount of blood loss,total operation time and first postoperative exhaust time in the treatment group were better than those in the control group(P<0.05).Conclusion Compared with the clinical effect of transabdominal total hysterectomy,the overall effect of laparoscopic total hysterectomy is more prominent,and all indexes are significantly better,which is suitable for widespread use.
