观察低相对分子质量肝素预防高龄产妇剖宫产术后下肢深静脉血栓形成的效果

摘　　要：目的分析高龄剖宫产产妇应用低相对分子质量肝素(LMWH)预防术后下肢深静脉血栓形成(DVT)的临床效果。方法抽取76例高龄剖宫产产妇为研究对象,按照随机数字表法分为对照组与治疗组,每组38例。对照组应用常规治疗,治疗组在对照组基础上联合应用LMWH治疗。对比两组术后DVT发生率、并发症发生率及血液指标变化情况。结果治疗组产妇术后DVT发生率为2.63%,低于对照组的15.79%(P<0.05);治疗组术后并发症发生率为5.26%,低于对照组的21.05%(P<0.05);治疗前,两组D二聚体(D-D)、血小板计数(PLT)及红细胞比容(HCT)对比,差异无统计学意义(P>0.05);治疗后,治疗组D-D、PLT、HCT水平低于对照组(P<0.05)。结论对高龄剖宫产产妇术后给予LMWH治疗可有效预防术后DVT发生,改善血液指标,降低并发症发生率。Objective To analyze the clinical effect of low molecular weight heparin(LMWH)in the prevention of postoperative deep vein thrombosis(DVT)in elderly women after cesarean section.Methods 76 elderly women with cesarean section were selected as research objects,and were divided into control group and treatment group by random number table,38 cases in each group.The control group received conventional treatment,and the treatment group was combined with LMWH treatment on the basis of the control group.The incidence of DVT,complication rate and blood index were compared between the two groups.Results The incidence of DVT in the treatment group was 2.63%,lower than that in the control group 15.79%(P<0.05).The incidence of postoperative complications was 5.26%in the treatment group,which was lower than 21.05%in the control group(P<0.05).Before treatment,there were no significant differences in D-dimer(D-D),platelet count(PLT)and erythrocyte specific volume(HCT)between the two groups(P>0.05).After treatment,the levels of D-D,PLT and HCT in the treatment group were lower than those in the control group(P<0.05).Conclusion LMWH treatment after cesarean section can effectively prevent DVT,improve blood indexes and reduce the incidence of complications.
