腹部超声联合阴道超声诊断子宫腺肌病的临床价值

摘　　要：目的研究腹部超声联合阴道超声诊断子宫腺肌病的临床价值。方法选取37例子宫腺肌病患者,所有患者均经病理学检查确诊,且于入院时接受了腹部超声、阴道超声检查,以病理学检查结果为金标准,比较腹部超声、阴道超声单一检查与联合检查对子宫腺肌病的检出率及漏诊率。结果以病理学检查结果为金标准,37例子宫腺肌病患者,腹部超声、阴道超声联合检查检出36例,检出率为97.30%,漏诊1例,漏诊率为2.70%;阴道超声检查检出31例,检出率为83.78%,漏诊6例,漏诊率为16.22%;腹部超声检查检出23例,检出率为62.16%,漏诊14例,漏诊率为37.84%。腹部超声联合阴道超声检查对子宫腺肌病的检出率高于阴道超声、腹部超声单一检查,漏诊率低于阴道超声、腹部超声单一检查,差异有统计学意义(P<0.05);阴道超声检查对子宫腺肌病的检出率高于腹部超声检查,漏诊率低于腹部超声检查,差异有统计学意义(P<0.05)。结论相较于单一检查,腹部超声联合阴道超声诊断子宫腺肌病的临床价值更高,可提高疾病检出率,降低疾病漏诊率。Objective To study the clinical value of abdominal ultrasound combined with vaginal ultrasound in the diagnosis of adenomyosis.Methods A total of 37 patients with adenomyosis were selected,all of whom were confirmed by pathological examination and received abdominal ultrasound and vaginal ultrasound on admission.Pathological examination results were used as the gold standard to compare the detection rate and missed rate of single and combined abdominal ultrasound and vaginal ultrasound for adenomyosis.Results According to the results of pathological examination as the gold standard,of 37 patients with adenomyosis,36 cases were detected by abdominal ultrasound and vaginal ultrasound,the detection rate was 97.30%,and 1 case was missed,the missed diagnosis rate was 2.70%.31 cases were detected by vaginal ultrasound,the detection rate was 83.78%,and 6 cases were missed,the missed diagnosis rate was 16.22%.23 cases were detected by abdominal ultrasound,the detection rate was 62.16%,and 14 cases were missed,the missed diagnosis rate was 37.84%.The detection rate of adenomyosis by abdominal ultrasound combined with vaginal ultrasound was higher than that by vaginal ultrasound and abdominal ultrasound alone,but the rate of missed diagnosis was lower than that by vaginal ultrasound and abdominal ultrasound alone,the differences were statistically significant(P<0.05).The detection rate of adenomyosis by vaginal ultrasound was higher than that by abdominal ultrasound,but the rate of missed diagnosis was lower than that by abdominal ultrasound,the difference was statistically significant(P<o.05).Conclusion Compared with single examination,abdominal ultrasound combined with vaginal ultrasound has higher clinical value in the diagnosis of adenomyosis,which can improve the detection rate of disease and reduce the rate of missed diagnosis ofdisease.
