超声引导三维立体水平式乳腺微创旋切术治疗乳房病损的临床疗效

摘　　要：目的 探讨超声引导三维立体水平式乳腺微创旋切术在乳房病损切除中的应用价值。方法 选取2020年8月至2022年3月宁夏医科大学总医院肿瘤外一科行乳房病损旋切术的155例患者为研究对象,以旋切方式不同将患者分为观察组(n=78)和对照组(n=77)。观察组行超声引导三维立体水平式乳腺旋切术,对照组行传统垂直式乳腺旋切术。比较两组患者手术相关指标、并发症、术后满意度评分及随访3、6、12个月病灶残留情况。结果 观察组手术时间短于对照组[(15.60±0.64) min vs.(21.16±0.87) min,P<0.05],术中出血量少于对照组[(13.50±3.51)mL vs.(16.90±2.62)mL,P<0.01];观察组术后并发症发生率低于对照组(2.56%vs.10.39%,P<0.05);观察组患者术后乳房对称性、凹陷程度、质地与弹性、皮肤颜色、乳头纵向偏移距离、乳房顺应性差值及总分均优于对照组,观察组术后满意度总分高于对照组(P均<0.05);而术后乳头横向偏移距离、瘢痕评分比较,差异均无统计学意义(P均>0.05)。结论 超声引导三维立体水平式乳腺微创旋切术具有缩短手术时间、减少术中出血、降低并发症发生率、降低术后病灶残留、术后患者满意度高等优点,值得在临床推广应用。Objective To investigate the application value of ultrasound-guided three-dimensional horizontal minimally invasive breast biopsy in the removal of breast lesions.Methods A total of 155 patients who underwent surgical resection of breast lesions from August 2020 to March 2022 in the First Department of Oncological Surgery of the General Hospital of Ningxia Medical University were selected as the research subjects.Patients were divided into observation group(n=78)and control group(n=77)by different surgical resection methods.The observation group was treated with ultrasonic probe cross guided horizontal minimally invasive mastectomy,while the control group was treated with traditional vertical mastectomy.Surgical indicators,complications,postoperative satisfaction score were compared between the two groups,and residual lesions were followed up for 3,6 and 12 months.Results The operation time of the observation group was shorter than that of control group[(15.60±0.64)min vs.(21.16±0.87)min,P<0.05].The amount of intraoperative blood loss was less than that of the control group[(13.50±3.51)mL vs.(16.90±2.62)mL,P<0.01].The incidence of postoperative complications in the observation group were lower than those in the control group(2.56%vs.10.39%,P<0.05).Postoperative breast symmetry,degree of depression,texture and elasticity,skin color,longitudinal deviation distance of nipple,difference of breast compliance and total scores in observation group were better than those in control group,and postoperative satisfaction scores in observation group were higher than those in control group(P all<0.05).There was no significant difference between the observation group and the control group in the transverse deviation distance and scar score after surgery(P all>0.05).Conclusion Ultrasound-guided three-dimensional horizontal Minimally Invasive Breast Biopsy has the advantages of shortening the operation time,reducing intraoperative bleeding,reducing the incidence of complications,reducing postoperative residual lesions,and hi
