马兰草凝胶贴剂对肝硬化腹水大鼠及其活化肝星状细胞凋亡的影响研究

摘　　要：目的探讨马兰草凝胶贴剂对肝硬化腹水大鼠及其活化肝星状细胞凋亡的影响。方法将雄性SD大鼠随机分为空白组、模型组、基质组、马兰草凝胶贴剂组和呋塞米+螺内酯组,每组10只。除空白组外,其余各组采用腹腔注射CCl 4-橄榄油混合液制备肝硬化腹水模型大鼠。造模成功后,基质组、马兰草凝胶贴剂组分别给予不含药物的基质贴和马兰草凝胶贴,每两天1次,每次1.5小时,持续8周;呋塞米+螺内酯组给予呋塞米+螺内酯混合液灌胃,每两天1次,持续8周。给药期间观察大鼠一般状态,测量腹水量、白蛋白、胶体渗透压变化。8周后麻醉处死大鼠,取材进行检测,苏木精—伊红染色法行肝组织病理学检测,Tunel-α-平滑肌激动蛋白(α-smooth muscle actin,α-SMA)双标记法检测活化肝星状细胞凋亡,免疫荧光法检测各组大鼠B淋巴细胞瘤-2(B-cell lymphoma-2,Bcl-2)和Bcl-2关联X(Bcl-2 associated X,Bax)蛋白表达。结果马兰草凝胶贴剂组有效改善腹水大鼠的一般状态,腹水量显著降低,血清白蛋白、血浆胶体渗透压水平明显升高,肝细胞结构接近正常,细胞大小均匀,门管区炎性细胞浸润显著减轻,α-SMA阳性细胞表达明显减少,荧光减弱,Tunel阳性细胞表达增多,Bax表达显著升高,Bcl-2表达显著下降。结论马兰草凝胶贴剂对肝硬化腹水大鼠腹水具有治疗作用,其作用机制可能与修复肝组织病理形态和促进活化的肝星状细胞凋亡有关。Objective To investigate the effect of Ammophila Arenaria gel patch on apoptosis of hepatic stellate cells in cirrhotic rats with ascites.Methods Male SD rats were randomly divided into blank group,model group,matrix group,Ammophila Arenaria gel patch group and furosemide+spironolactone group.Except blank group,the other groups were intraperitoneally injected with CCl 4-olive oil mixture to prepare cirrhotic ascites model rats.Matrix group and malan herb gel patch group were given drug-free matrix patch and Ammophila Arenaria gel patch,once every two days for 1.5h each time,for 8 weeks,while furosemide+spironolactone group was given furosemide+spironolactone mixture intragastric administration,once every two days for 8 weeks.8 weeks later,anesthesia was used for sampling.The general state of rats was observed,the changes of abdominal water volume,albumin and colloid osmotic pressure were measured.followed by HE of histopathological detection.Tunel-α-SMA method was performed to detect the apoptosis of activated HSCs.Immunofluorescence were used to detect the protein expression of Bax and Bcl-2 in hepatic tissue.Results Ammophila Arenaria gel patch group can effectively improved the general state of asitoneal rats,decreased significantly of abdominal water volume,serum albumin and plasma colloid osmotic pressure,increased significantly of liver cell structure was close to normal,cell size was uniform,inflammatory cell infiltration in portal area decreased significantly,α-SMA positive cells expression decreased significantly,fluorescence decreased.Tunel positive cells expression increased.The expression of Bax was significantly increased,while the expression of Bcl-2 was significantly decreased.Conclusion Ammophila Arenaria gel patch has therapeutic effect on the ascites of cirrhotic rats.Its mechanism may be related to repairing the pathological morphology of liver tissue and promoting the apoptosis of activated hepatic stellate cells.
