长沙地区血小板供者基因数据库理论建模分析

摘　　要：目的分析长沙地区血小板供者基因数据库建设情况,评估为临床提供HLA/HPA匹配的单采血小板的可能性。方法分析长沙地区血小板供者基因数据库1029例无亲缘关系单采血小板供者HPA、HLA⁃A/B等位基因及抗原分布情况,评估长沙地区血小板供者基因数据库建设规模。结果在不考虑ABO血型的情况下,当库容为207人时,约有95%的患者能在该库中找到至少1名HPA基因型全匹配的供者;当库容为2127人时,约有95%的患者能在该库中找到至少1名HLA⁃A/⁃B表型匹配的供者;当库容为1029人时,约有79.25%的患者能在该库中找到至少1名HLA⁃AB表型匹配的供者。单采血小板捐献者A、B抗原CREG表位不配合的概率最高分别为1C⁃2C和7C⁃5C,Broad CREG不配合概率最高的为4C⁃6C。结论长沙地区已建立已知HLA/HPA基因型的血小板供者基因数据库库容1029人,在扩大有效库容的同时将进一步验证理论建模的适用性和匹配概率的符合性。Objective To analyze the construction of platelet donor gene database in Changsha area,and evaluate the possibility of providing HLA/HPA⁃matched apheresis platelets for the clinic.Methods The distributions of HPA and HLA⁃A/B alleles and anti⁃gens in 1029 unrelated and random platelet donors in Changsha area were analyzed,and the construction scale of platelet donor gene database in Changsha area was evaluated.Results Without considering the ABO blood types,it was deduced that when the platelet donor database had a capacity of 207 persons,about 95%of patients could find at least one HPA genotype fully matched donor in the database.When the database had a capacity of 2127 persons,about 95%of patients could find at least one HLA⁃A and⁃B phenotype fully matched donor in the database.When the database had a capacity of 1029 persons,about 79.25%of patients could find at least one HLA⁃AB phenotype fully matched donor in the database.The highest mismatch probabilities of CREG epitopes of A and B antigens in apheresis platelet donors were 1C⁃2C and 7C⁃5C,respectively.The highest mismatch probability of Broad CREG was 4C⁃6C.Con⁃clusion A platelet donor gene database with known HLA/HPA genotypes has been established in Changsha area,with a capacity of 1029 persons.While expanding the effective capacity,the applicability of theoretical modeling and conformity of matching probability will be further verified.
